RF、CPA、APF之间的相关性及联合检测在类风湿性关节炎诊断中的意义

摘　　要：目的探讨类风湿因子(RF)、抗瓜氨酸合成蛋白抗体(CPA)、抗核周因子(APF)三者间的相关性及联合检测在类风湿性关节炎(RA)诊断中的意义。方法对191例RA患者血清RF检测采用胶乳凝集法,CPA检测采用酶联免疫吸附实验法(ELISA),APF检测采用间接免疫荧光法(IIF)。结果 RF与APF、CPA无明显相关性(P>0.05),APF与CPA有相关性(P<0.05)。在RA患者中,RF检测的阳性率为45.5%,CPA检测的阳性率为53.5%,APF检测的阳性率为56.4%,联合检测的阳性率达87.9%。经统计学分析联合检测与单项检测比较有高度统计学意义(P<0.01)。结论 APF与CPA有相关性,RF、APF、CPA三种试验联合检测可明显提高RA的诊断率。Objective To explore the correlation of rheumatoid Factor（RF）,citrullinated protein IgG antibody（CPA）and the antiperinuclear factor（APF）and the significance of their combination in the diagnosis of rheumatoid arthritis（RA）.Methods We select 191 serum samples from patients with RA,and make the latex agglutination test on RF,enzyme linked immunosorbent assay（ELISA） on CPA and Indirect immunofluorescence（IIF） on APF.Results There is no obvious correlation of RF with CPA and APF（P0.05）;there is a correlation between CPA and APF（P0.05）.The detections on the patients with RA,the positive detection rate of RF,CPA and APF is respectively 45.5%,53.5% and 56.4%.The joint detection rate is 87.9%.Compared with the other three specific detections,the joint detection has more statistical significance（P0.01）.Conclusion There is a correlation between APF and CPA.The combine of RF,CPA and APF can significantly improve the diagnosis rate of RA.
