Ommaya囊置入术在神经系统疾病治疗中的应用价值

摘　　要：目的探讨Ommaya囊置入术在神经系统疾病治疗中的应用价值。方法回顾性收集2006年1月—2011年1月在我院神经内、外科住院行Ommaya囊置入术的84例患者的病历资料,对患者的疾病谱、病情转归及术后并发症进行分析。结果 84例患者中脑室出血24例,蛛网膜下腔出血20例,中枢神经系统肿瘤16例,梗阻性脑积水14例,中枢神经系统感染9例,因嗜铬细胞瘤继发高血压脑病致可逆性后部白质脑病综合征1例。Ommaya囊置入术后死亡18例,转行分流术26例,取出Ommaya囊18例,继续留置Ommaya囊22例。术后并发颅内感染8例(9.5%)、引流管堵塞3例(3.6%)。结论 Ommaya囊置入术可用于治疗因各种病因引起的颅内压异常升高,同时可以提供局部给药的途径,在临床上具有重要应用价值。Objective To investigate the effectiveness of Ommaya reservoir（OR） as a treatment method for centre nervous system diseases.Methods Eighty-four patients admitted to our hospital between Jan.2006 and Jan.2011 were retrospectively reviewed with the basis disease,treatment effects and postoperative complications analyzed.Results Of the 84 cases with centre nervous system diseases,24 were with ventricle hemorrhage,20 were with subarachnoid hemorrhage,16 were with tumors,14 were with hydrocephalus,9 were with meningitis,and the rest one were with reversible posterior leukoeneephalopathy syndrome.After the operation,the reservoir was removed in 18 cases for the ameliorated clinical symptoms.Twenty-six cases needed CSF shunt for long-term and underwent ventricle-abdomen shunt in several months.Eighteen cases died after the operation.Ommaya reservoir were reserved in the rest 22 cases.Intracranial infection occurred in 8 cases（9.5%）,and obstruction was found in 3 cases（3.6%）.Conclusion The placement of Ommaya reservoir is among the safe and effective methods for the treatment of many centre nervous system diseases.Because it can be implanted in emergency and provide a route for drug administration,Ommaya reservoir insertion is a potential technique in the first aid of uncontrollable intracranial hypertension.
