玻璃体切除联合硅油填充术治疗外伤性睫状体分离

摘　　要：目的:探讨玻璃体切除联合硅油填充在睫状体分离中的临床治疗效果。方法:回顾性分析2003年3月-2008年3月于我院确诊的22例外伤性睫状体分离患者的临床资料,所有患者均行玻璃体切除联合硅油填充术。术后4-5个月行硅油取出术。结果:19例患者术后眼压恢复正常,17例患者术后视力明显提高,5例患者术后第7天超声生物显微镜(UBM)示"睫状体分离,分离程度较术前减轻"(眼压正常)。术后第1个月复查UBM,示"睫状体复位"。其余患者UBM检查均提示"睫状体复位"。术后4-5个月所有患者行硅油取出术,取出后行视力、眼压及UBM检查,16例患者术后视力无明显变化,20例患者术后眼压正常,2例患者术后眼压低,予以激素治疗后恢复正常。术后第7天、3个月复查UBM示"睫状体在位"。结论:玻璃体切除联合硅油填充术在治疗外伤性睫状体分离,特别是伴有其他严重眼外伤者,具有优势。Objective： To explore the treatment of cyclodialysis with pars plana vitrectomy combined with silicone oil tamponade.Methods： A total of 22 patients diagnosed of traumatic cyclodialysis treatment underwent vitrectomy combined with silicone oil tamponade which was taken out 4-5 months later.Results： The intraocular pressure after surgery was normal in 19 patients,but it was higher in 3 patients and went down to normal after conservative treatments.Improvement of visual acuity was observed in 17 patients,while unchanged or a little decreased visual acuity was found in 5 patients.Ultrasound biomicroscopy（UBM） on the 7th day showed ciliary body reset in 17 cases,and cyclodialysis in the other five cases but with decreased degrees and normal intraocular pressure in 5 patients,and ciliary body reset was shown till one month later.After 4-5 months,all of them underwent silicone oil taken out.Unchanged visual acuity was observed in 16 patients,and a little decreased visual acuity was in 6 patients.The intraocular pressure of 20 patients was normal after surgery while that of 2 patients was lower,which went up to normal after conservative treatment.UBM was also done on the 7th day and 1 month later,and showed all cases had normal ciliary bodies.Conclusion： It＇s a recommended method to treat traumatic cyclodialysis,especially accompanied with other severe complications,with vitrectomy combined with silicone oil tamponade.
