摘 要:目的:检测直肠癌中血管内皮生长因子(VEGF)及血管生成素-2(Ang-2)的表达,探讨两者与直肠癌临床病理因素和肿瘤新生血管形成之间的关系。方法:采用免疫组化SP法检测60例直肠癌、30例直肠腺瘤及10例直肠正常黏膜组织中VEGF、Ang-2及FⅧ标记的微血管密度(MVD)表达,分析不同VEGF和Ang-2表达状态下直肠癌组织MVD的变化。结果:在直肠癌、直肠腺瘤及正常组织中VEGF的阳性表达率分别为71.7%(43/60)、43.3%(13/30)和30.0%(3/10),Ang-2的阳性表达率分别为73.3%(44/60)、46.7%(14/30)和30.0%(3/10)。VEGF的表达与患者的性别和年龄、肿瘤的形态和肿瘤部位、术前CEA情况及肿瘤分化程度无关(P>0.05),与直肠癌临床分期、淋巴结转移及远处转移相关(P<0.05);Ang-2的表达与患者的性别和年龄、肿瘤的形态和肿瘤部位、术前CEA情况及淋巴结转移无关(P>0.05),与肿瘤分化程度、直肠癌临床分期及远处转移相关,P<0.05。直肠癌组织中VEGF和Ang-2的表达呈正相关,r=0.290,P<0.05。VEGF在直肠癌组中染色阳性者其MVD较染色阴性者高(11.04±1.68 vs 7.66±1.22,t=7.53,P<0.05);Ang-2染色阳性者其MVD较染色阴性者高(10.97±1.73 vs 7.66±1.31,t=6.95,P<0.05)。结论:VEGF和Ang-2高表达与直肠癌发生发展及侵袭转移密切相关,在调节直肠癌的血管生成中可能起协同作用。OBJECTIVE: To investigate the expressions of VEGF and Ang-2 in rectal cancer and its relationship with clini copathlolgic factors and mcrovessel density (MVD) in tumor tissue. METHODS: VEGF, Ang-2 and factor Ⅷ related antigen were stained immunohistochemically in 60 cases of primary rectal cancer,30 cases of rectal adenoma and 10 cases of normal rectal mueosal tissue,and MVD was also assayed in above tissue speci mens. RESULTS: The positive expression rates of VEGF in rectal carcinoma tissues, rectal adenoma tissues and normal tissues were 71.7% (43/60), 43. 3% (13/30) and 30. 0% (3/10) respectively, while the positive expression rates of Ang-2 were 73.3% (44/60), 46.7% (14/30), 30. 0% (3/10). The positive expression of VEGF did not have correlations with gender, age, tumor morphology and lo- cation,CEA,differentiation (P〉0.05) ,but had correlation with ree tal cancer clinical stage,lymph node metastasis and metastasis from blood vessels (P〈0. 05). The positive expression of Ang-2 did not have correlations with gender, age, tumor morphology and location, CEA,lymph node metastasis (P〈 0.05), but had correlation with differentiation,rectal cancer clinical stage and metastasis from blood vessels (P〈0. 05) ,The high positive expression of VEGF and Ang- 2 had a significant positive correlation in rectal cancer (r =0. 290, P〈0. 05). The MVD in cases of the VEGF- positive group was higher compared to the negative group(11. 04± 1.68 vs 7.66± 1.22 ,t= 7.53 ,P〈 0. 05), the MVD in cases of the Ang-2 positive group was higher than that of the negative group(10.97 ± 1.73 vs 7.66±1.31, t = 6.95, P〉0. 05). CONCLUSION.. The overexpres sion of VEGF and Ang-2 may be related to the development, invasion and metastasis of rectal cancer, and they may play synergistic role in angiogenesis.
