摘 要:目的对金属夹在内镜下胃肠大息肉电切除术中的应用价值进行评估。方法内镜下高频电切除胃肠息肉86例,对32例息肉头体部最大直径〉2.0cm,蒂径〉1.0cm者应用HX-610-135金属夹钳夹蒂部后行电切除或电切除后立即夹闭创面(治疗组)。对54例息肉头体部最大直径1.0~3.0cm,蒂径0.5~1.2cm者行单纯高频电切除(对照组)。观察两组治疗过程中和治疗后出血、穿孔等并发症情况。结果治疗组并发症发生率显著低于对照组(P〈0.05)。结论内镜下高频圈套电切较大胃肠息肉预防性应用金属夹能够显著降低其术中及术后出现的并发症,明显提高手术安全性。Objective To evaluate the value of endoscopic metal clips in resection of large gastrointestinal polyps. Methods Endoscopic polypectomy using high frequency current was performed for 86 cases of gastrointestinal polyps. Treatment group: 32 cases with the head diameter of the polyp greater than 1.5 cm and pediele diameter greater than 1.0 cm were either hemoclipped using HX -610 - 135 followed by using high frequency current for resection of polyps or resected by high frequency current followed immediately by clipping the wound site. Control group: 54 cases with the head size of the polyp between 1.0 - 3.0 cm and pedicle between 0.5 - 1.2 cm in diameter underwent polyp resection by high frequency current only. Complications such as bleeding and perforation were compared between the two groups. Results Complications in the treatment group were significantly lower than that in the control group( P 〈 0.05 ). Conclusions Prophylactic application of metal clips in endoscopic polypectomy utilizing high frequency current to remove large gastrointestinal polyps might significantly improve surgical safety and reduce the intraoperative and postoperative complications.
