内生场热化疗在预防膀胱尿路上皮癌复发中的疗效

摘　　要：目的降低膀胱尿路上皮癌术后复发率，评价内生场热化疗联合应用的疗效和安全性。方法对20例膀胱尿路上皮癌术后患者，术后24h内行丝裂霉素20mg／20ml膀胱灌注1次，术后1个月开始在膀胱内注入丝裂霉素20mg／100ml，采用仰卧位和俯卧位各10min后，内生场膀胱区热疗1h，每月1次，每3个月行膀胱镜检查。若1年无复发，改为每2个月1次热化疗，半年复查1次膀胱镜，1年后无复发可选择性膀胱灌注，长期随访，总疗程为2年，丝裂霉素总灌注量360mg。结果20例患者中1例失访，余19例均获随访，随访3～32个月，除1例术后9个月复发外，余18例均未见肿瘤复发及转移，复发率为5．26％。结论内生场热化疗联合应用预防膀胱肿瘤术后复发效果满意，安全性好。Objective To evaluate the efficacy and safety of endogenous field thermal chemotherapy for the prevention of recurrence of urothelial carcinoma of bladder after surgery. Methods Twenty cases of urothelial carcinoma of bladder who underwent surgical management received bladder perfusion with mitomycin in 24 hours bostoperatively, and one month postoperatively received bladder perfusion with mitomyeim 20 rag, supined and proned every 10 minutes, and then received endogenous field thermal chemotherapy for one hour, monthly, and received the bladder endoscopy every three months. If no recurrence oceured after one year, then ehanged to bimonthly, and received bladder endoscopy every half a year. Total treatment was for two years, total dose was 360 rag. Results All 20 cases except one were followed up for 3 months to 32 months. One case got recurrence within 9 months. The recurrence rate was 5.26%. No obvious side effect was found in all cases. Conclusions Endogenous field thermal chemotherapy is effective for preventing the recurrence of urothelial carcinoma of the bladder after surgical management.
