胃癌组织中EZH2和p53蛋白的表达及临床意义

摘　　要：目的探讨EZH2和p53蛋白在胃癌组织中的表达,及其在胃癌发生、发展过程中的作用和临床病理学意义。方法采用免疫组化EnVision法检测EZH2和p53蛋白在199例胃癌、25例高级别上皮内瘤变、24例低级别上皮内瘤变、46例肠上皮化生、17例慢性萎缩性胃炎和23例正常胃黏膜组织中的表达情况。结果在胃癌和上皮内瘤变组织中EZH2蛋白表达明显高于肠上皮化生、慢性萎缩性胃炎和正常胃黏膜组织。胃癌组织中EZH2蛋白的表达在进展期胃癌高于早期胃癌,差异有统计学意义。EZH2蛋白在胃癌组织中的表达与肿瘤的大小、淋巴结转移、组织学分期和临床分期均有关;但与患者的性别、年龄、肿瘤的部位、肿瘤的类型和分化以及患者的病死率均无关。p53蛋白在胃癌组织中表达最高,几乎不表达于其他病变和正常胃黏膜组织。p53表达水平与患者的年龄、性别、肿瘤大小、淋巴结转移、肿瘤分化、胃癌的类型和患者的生存情况相关。EZH2蛋白表达与p53蛋白表达之间呈正相关。结论 EZH2在胃癌中的表达增加与p53相关,提示胃癌中EZH2与p53可能相互协调,促进胃癌的发生、发展。Purpose To investigate the expressions of EZH2 and p53 protein in gastric carcinomas,and then to determined the clinicopathological significance of both proteins in the carcinogenesis and progression of gastric carcinoma.Methods 199 cases of gastric carcinomas,25 of high-grade intraepithelial neoplasia,24 of low-grade intraepithelial neoplasia,46 of intestinal metaplasia,17 of chronic atrophic gastritis and 23 of normal gastric mucosa were immunohistochemically analysed with EZH2 and p53 protein.Results The expression of EZH2 protein in gastric carcinomas and intraepithelial neoplasia was significantly higher than that in intestinal metaplasia,chronic atrophic gastritis and normal gastric mucosa. Meanwhile,the expression of EZH2 protein was higher in advanced gastric cancer than that in early gastric cancer.The expression of EZH2 protein was correlated with tumor size,lymph node metastasis,T stage and clinical stage.However,the expression of EZH2 protein was not correlated with the patients＇ age,gender,tumor location,tumor differentiation,tumor classification and patients＇ survival.The expression of p53 protein was greatly enhanced in gastric cancer.In other lesions and normal gastric mucosa,the expression of p53 was completely absent or rare.The expression level of p53 was correlated with patients＇ age,gender,tumor size,lymph node metastasis,tumor differentiation,tumor classification and patients＇ survival.EZH2 protein expression was positively correlated with p53 protein expression.Conclusion EZH2 is overexpressed in gastric cancer and the expression is correlated with the expression of p53,which suggested that crosstalk between EZH2 and p53 may play an important role in carcinogenesis and progression of gastric carcinoma.
