丙戊酸镁缓释片与碳酸锂对躁狂发作的疗效及认知功能的对照研究

摘　　要：目的比较丙戊酸镁缓释片与碳酸锂对躁狂发作患者的疗效及对认知功能的影响。方法采用入院顺序分层随机法,将120例躁狂发作患者平均分为研究组(丙戊酸镁缓释片)和对照组(碳酸锂),在治疗前,治疗后4、8、12周末各做1次韦氏成人智力量表、韦氏记忆量表、铁槽铁钉测验、手指敲击试验、动作功能测验、手功能协调测验、连线测验a和b、威斯康星卡片分类测验(WCST)及言语流利性测验等10项神经心理测查及B ech-R afaelsen躁狂量表(BRMS)和临床疗效总评量表(CGI),以副反应量表(TESS)评定疗效和副作用,韦氏记忆量表(WMS)评定治疗前后患者认知功能的改变。结果治疗12周后,两组BRM S总分与治疗前比较有显著性差异(t=2.87,P<0.01),各因子分比治疗前明显降低(t=2.94,P<0.01),研究组有效率98.3%,显效率71.7%;对照组有效率96.7%,显效率68.3%。两组间疗效无显著性差异(t=0.38,P>0.05),碳酸锂组副作用比丙戊酸镁缓释片组多。经12周治疗,从各项神经心理测查均值的变化趋势看,研究组与对照组从8周开始有逐渐好转的趋势,与治疗前相比,敲击测验,动作能力测验,手功能协调测验,WCST完成类别数等认知功能均显著改善,且研究组好于对照组(t=1.82,P<0.05)。结论丙戊酸镁缓释片治疗躁狂发作疗效好,副作用小,对认知功能的改善更彻底。】Objective To observe the efficacy and cognition function of Magnesium Valproate Retard Tablets and lithium carbonates in the treatment of manic episode.Methods A total of 120 manic episode patients were divided into study group（Magnesium Valproate Retard Tablets）and control group（lithium carbonates） randomly.Before treatment and at the end of 4,8,12 weeks,Wechsler adult intelligence scale,Wechsler memory scale,grooved pegboard test,finger tapping test,motor function test,hand coordination test,trail making test A and B,Wisconsin card sort test,and verbal fluency test were carried out,Effects and side effects were assessed with BRMS,CGI and TESS respectively.Results After 12-week treatment,there were significant differences in BRMS total scores between before and after treatment for both groups（t=2.87,P0.01）,and each factor score declined significantly（t=2.94,P0.01）,in study group,98.3% was efficient,71.7% was efficiency;in control group 96.7% efficient and 68.3% efficiency.There was no significant difference between them（t=0.38,P0.05）,side effect ratio in control group was higher than that in study group.Through 12-week treatment,there was gradual amendment of trend after 8 weeks,and significant differences in pound test,action ability test,hand function moderate test,WCST completion category number were found between them.Conclusion Magnesium valproate retard tablets is effective to treating manic episode,weaker side effects,and improving cognition function.
