听神经瘤术中面神经监测的意义

摘　　要：目的 在听神经瘤手术中行面神经监测,探讨术中监测的意义.为听神经瘤的临床治疗及预后提供帮助.方法 选取142例经手术治疗的听神经瘤患者,其中,50例行术中面神经监测,术后观察面神经解剖及功能保留情况并与其余92例未行术中监测的患者进行比较.结果 ①50例术中行面神经监测患者,面神经解剖保留40例,保留率80.0%,未行监护患者面神经解剖保留57例,保留率62.0%,两者之间呈显著差异（P〈0.05）.②50例术中面神经监测患者,面神经功能保留23例,保留率46.0%,未行监测患者面神经功能保留26例,保留率28.3%,两者之间呈显著差异（P〈0.05）.结论 听神经瘤术中行面神经监测,可以有效提高术后面神经解剖、功能保留率.Objective Monitoring facial nerve in the operations of acoustic neuroma, and to explore its significance so as to offer help for clinical treatment. Methods One hundred and forty two patients with acoustic： neuroma treated with surgery were selected. The monitoring of facial nerve was performed in 50 patients and the remaining 92 cases were without monitoring. The rates of facial nerve anatomy and function reservation between the two groups were com- pared. Results ①Forty cases had facial nerve anatomy reservation in group with intraoperative monitoring, the reservation rate was 80.0% ;and 57 cases in group without intraoperative monitoring, the rate was 62.0%. The difference was statistical significantly（ P 〈 0.05 ）. @Twenty-three cases had facial nerve function reservation in group with intraoperative monitoring, the resmwation rate was 46.0% ; and 26 cases in the group without intraoperative monitoring, the rate was 28.3%. The difference was statistical significantly（ P 〈 0.05 }. Conclusion The technique of intraoperative monitoring can effectively impove facial nerve anatomical and functional preservation rates in surgery of acoustic, neuronm.
