腹腔内区域性温热化疗联合力尔凡治疗胃肠道肿瘤

摘　　要：目的探讨腹腔内区域性温热化疗联合力尔凡治疗胃肠道肿瘤的治疗效果。方法 116例胃肠道肿瘤切除术后病人随机分为治疗组(62例)和对照组(54例),两组均行术中序贯腹腔温热灌洗联合术后静脉微泵化疗3次和超选择置泵持续动脉区域性灌注化疗3次,治疗组在化疗当天开始辅助力尔凡治疗;比较两组患者腹腔游离癌细胞的检测阳性率、腹腔转移和肝转移率及3年生存率。结果①游离癌细胞阳性率:胃癌对照组和治疗组术前分别为55.5%和50.0%,两者比较差异无统计学意义(P>0.05),术后分别为44.4%和18.1%,两者比较差异有统计学意义(P<0.05)。直肠癌对照组和治疗组术前分别为50.0%和47.5%,两者比较差异无统计学意义(P>0.05),术后分别为47.2%和17.5%,两者比较差异有统计学意义(P<0.05)。②肿瘤复发或转移:对照组发生率为31.4%,治疗组为17.7%,两者比较差异有统计学意义(P<0.05)。③生存率:对照组1年和3年生存率分别为67.3%和48.9%,治疗组1年和3年生存率分别为84.4%和79.3%,两者比较差异有统计学意义(P<0.05)。结论术中腹腔温热灌洗术后静脉微泵和超选择置泵持续动脉区域性灌注联合力尔凡辅助治疗对胃肠道肿瘤病人术后腹腔及肝转移有良好的防治作用。Objective To study Lifein combined with the sequential optimized program of chemotherapy during intraoperative and postoperative gastrointestinal cancer resection,in order to effectively prevent the peritoneal and hepatic recurrence.Methods 116 cases of gastrointestinal cancer patients were divided into treated group（62 cases） and control group randomly（54 cases）.Two groups all treat by intraperitoneal hyperthermic perfusion chemotherapy and postoperative continuous intravenous micro-pump chemotherapy three times and super-selective placement of pump arterial infusion chemotherapy three times,but treat group combined with Lifein at the begining of the chemotherapy.The peritoneal and hepatic metastasis rates and 3-year survival were studied.Results The positive rate of free cancer cells,the control group and treatment group in gastric cancer were preoperatively 55.5%,50.0%（P〈0.05）and 44.4%,18.1% postoperatively（P〈0.05）.The control group and treatment group in rectum cancer were preoperatively 50.0%,47.5%（P〈0.05） and 47.2%,17.5% postoperatively（P〈0.05）.The peritoneal and hepatic metastasis rates.The incidence of the control group and treatment group were 31.4%,17.7% respectively（P〈0.05）.3-year survival rates,the 1,3-year survival rates in control group were 67.3%,48.9% respectively and treatment group were 84.4%,79.3%respectively（P〈0.05）.Conclusion Intraoperative intraperitoneal hyperthermic perfusion chemotherapy combined with Lifein and postoperative continuous intravenous micro-pump chemotherapy and super-selective placement of pump arterial regional infusion chemotherapy is an effective way to control the recurrence on the peritoneal and hepatic metastasis of advanced gastrointestinal neoplasms after operation.
