中药联合FTS抗大鼠梗阻性黄疸肝脏缺血再灌注损伤的研究

摘　　要：目的:观察中药联合快速康复外科(FTS)对肝脏缺血再灌注损伤的疗效。方法:健康SPF级SD大鼠60只,随机分成假手术组、非FTS组、FTS常规组及FTS+中药组,各15只。所有大鼠均行胆总管结扎制备为梗阻性黄疸模型,FTS组及非FTS组均进行肝脏缺血再灌注操作。FTS+中药组加用中药干预。24h后检测天门冬氨酸氨基转移酶(AST)、谷氨酸氨基转移酶(ALT),HE染色观察大鼠肝脏形态学改变,TUNEL法检测肝组织细胞原位凋亡,并计算凋亡指数(AI)。结果:各组大鼠血清ALT、AST水平,假手术组与FTS组比较差异无统计学意义(P>0.05),与非FTS组比较,差异有统计学意义(P<0.05);非FTS组与FTS组比较,差异有统计学意义(P<0.05);FTS+中药组与FTS常规组比较,差异有统计学意义(P<0.05);形态学观察:FTS组肝脏损伤情况轻于非FTS组;假手术组凋亡指数显著低于其他各组(P<0.01),非FTS组高于FTS组(P<0.05)。结论:中药联合快速康复外科方法可降低梗阻性黄疸大鼠缺血再灌注损伤后肝细胞的损伤程度。objective： To observe the effect of fast-track surgery（FTS） combined with herbal medical therapy on hepatic ischemical reperfusion injury in rats with jaundice.Methods： A total of 60 SPF SD rats were randomly divided into the Sham,non-FTS,and FTS（general FTS and combined herbal medicine subgroups） groups,15 in each.All rats underwent bile duct ligation to eastablish the model of jaundice.In the non FTS and FTS groups,hepatic ischemical reperfusion injury was induced.In the FTS with herbal medicine group,herbal medicine was added.In 24 h after intervention,the levels of ALT and AST were measured and morphologic changes and apoptosis of hepatocytes were observed.Results： A significant difference in the levels of AST and ALT was noted among the groups（P0.05） except for that between the SHAM and the FTS groups（P0.05）.Apoptosis of hepatocytes was milder in the sham group than that in others（P0.01）;that was more severe in the non FTS group than that in the the FTS groups（P0.05） and no difference was noted between the general FTS subgroup and the FTS combined with herbal medicine group（P0.05）.Conclusion： FTS with herbal medicine may attenuate the injury of heaptocytes after ischemic-reperfusion injury.
