结直肠癌患者CEAmRNA表达与血清CEA及相关蛋白联合检测的临床意义

摘　　要：为探讨结直肠癌患者外周血癌胚抗原信使核糖核酸（CEAmRNA）的表达与血清癌胚抗原（CEA）联合CA19—9、CA242和CA724蛋白检测的临床意义，采用逆转录-聚合酶链反应（RT—PCR）和化学发光法，检测125例术前和96例术后结直肠癌患者及150名健康对照者外周血中CEAmRNA表达和血清CEA、CA19-9、CA242和CA724蛋白水平。结果结直肠癌患者血清CEA及相关蛋白联合检测的阳性率为79．1％，高于CEAmRNA（53．6％）和健康对照者（2．7％），有显著性差异（P〈0．01）。两种方法阳性检出率与肿瘤Dukes分期、淋巴结及脏器转移明显相关。血清CEA及相关蛋白联合检测阳性率在分化程度低的患者组中为74．3％，明显高于高分化组（52．8％），有显著性差异（P〈0．01），而CEAmRNA表达与肿瘤细胞分化程度无关。术后随访中发现CEAmRNA和血清CEA及相关蛋白水平明显增高的患者，均发生肝、肺和盆腔等远处转移。结果表明，外周血CEAmRNA和血清CEA联合CA19—9、CA242和CA724的检测可作为判断结直肠癌恶性程度、转移及监测疗效的重要指标，血清CEA及相关蛋白联合检测的敏感度优于外周血CEAmRNA表达。The objective of this study was to explore the clinical significance of the expression of CEA mRNA in blood of colorectal cancer patients, and combination detection of serum CEA with related proteins such as CA19-9, CA242 and CA724. By using reverse transcription-polymerase chain reaction（RT-PCR） and chemoluminescence test the expression of serum CEA mRNA and serum levels of CEA, CA19-9, CA242 and CA724 proteins in preoperative peripheral blood of 125 patients, in postoperative peripheral blood of 96 patients,and in peripheral blood of 150 health control subjects were detected respectively. As results,in patients positivity rate of serum CEA and combination detection of related proteins was 79.1 %, which was higher than 53.6% of CEA mRNA and 2.7% of health controls（ P 〈0.01） ;while detected ratio was significantly related to tumor Dukes staging,lymph node and organ metastasis;the positivity rate of serum CEA and combination detection of related proteins in low differentiation degree patients was significantly higher than that in high degree patients（74.3% vs 52.8%, P d0.01） ,but there was no relationship between the expression of CEA mRNA and the differentiation degree of neoplasm cell. Postoperative follow-up found that the patients having significantly increased levels of CEA mRNA, serum CEA and related proteins all suffer the remote metastasis to liver,lung and pelvic cavity,etc. Results show that the combination detection of peripheral blood CEA mRNA, serum CEA and CA19-9,CA242,as well CA724 can be served as the important indexes for judging the malignant extent,metastasis of colorectal carcinoma,and therapeutic effect on it;And the combination detection of serum CEA and related proteins has more sensitive than that of peripheral blood CEA mRNA expression.
