超声诊断外周神经鞘类肿瘤的临床价值

摘　　要：目的探讨超声诊断外周神经鞘类肿瘤的声像图特点及其临床应用价值。方法对41例经手术病理证实的外周神经鞘类肿瘤的声像图特征进行分析。结果 41例外周神经鞘类肿瘤中,33例为神经鞘瘤(颈部10例、上肢12例、下肢5例、背部3例、后腹膜3例),为单发性。图像特征以椭圆形实性低回声为主(30/33),边界清晰,有包膜,神经偏向一侧(11/33)。8例神经纤维瘤(上肢5例、下肢3例),6例单发者为实性低回声,内部回声尚均匀,呈梭形,两端与神经相连,2例为多发,呈串珠状。结论超声易发现臂丛神经及四肢神经干与神经肿瘤的关系,声像图的特征性表现可为临床决定手术方式提供影像学依据。Objective To explore the sonographic features anti clinical value of peripheral nerve sheath tumors by high fl＇equency ultrasound. Methods Fortyone patients with peripheral nerve sheath tumors were confirmed by surgery and pathology, then their sonographie features were analyzed. Results Among these 41 cases, 33 cases were ueurilemmomas （ 10 cases in the cervical part, 12 cases in the upper limbs, 5 cases in the lower limbs, 3 cases in the back, 3 cases in the peritoneum）. Most of these were oval hypoeehoic solid tumors （30/33） , with defined margins and capsules. The nerve turned be on one side （11/33）. 8 cases were neurofibromas （5 cases in the upper limbs, 3 cases in the lower limbs）. 6 cases of them were solitaire, with solid homogenous hypoechoic components, fusifonn in shape, with the nerve entering at one end, passing through and confing out at the other. The rest 2 eases were multiple tumors, like pearl-strings. Conclusion High frequency ultrasound can easily demonstrat that there is a certain relationship between the tumors and nerves in the braehial plexus and in the extremities. The characteristics of sonogram can help decide the best mode of surgery.
