摘 要:评价动态增强磁共振成像在早期乳腺癌诊断中的价值。随机选取2008年10月到2011年6月本院83例经B超或B超联合钼靶检查阴性的中青年女性行乳腺MRI检查,对MRI发现可疑结节病灶的患者进行手术活检,通过病理诊断来评价MRI在诊断早期乳腺癌的准确性和临床应用价值。83例中青年女性中,发现病变7例(8.4%),癌前改变4例(4.8%),癌3例(3.6%),其中仅行B超检查的58例,MRI发现1例乳腺导管内原位癌(ductal carcinoma in situ,DCIS),1例粘液癌,2例中重度非典型乳腺增生;B超和钼靶检查阴性的25例女性乳腺行MRI检查后,发现1例DCIS和2例重度非典型乳腺增生。3例癌中保乳手术2例,改良根治术1例,4例不典型增生者行腺叶切除术。对B超与钼靶检查或者两者联合检查阴性的中青年女性行乳腺MRI检查,可明显提高早期乳腺癌的检出率,从而有利于提高乳腺癌的保乳率。To evaluate dynamic contrast--enhanced magnetic resonance imaging(MRI) in early breast cancer diagnosis, 83 cases were randomly selected from October 2008 to June 2011 in our hospital examined by B--ultrasonic or B--ultrasonic combining mammography--negative young women implement breast MRI. Patients with suspicious nodules in MRI applied surgical biopsy and pathological diagnosis to evaluate the accuracy of MR/in the diagnosis of early breast cancer and clinical application. Among the 83 eases of young women, lesions were found in 7 patients (8.4% ) precancerous changes in 4 cases (4.8%) , carcinoma in 3 cases (3.6%), of which only B--ultrasonic examination of 58 cases, MRI found in 1 case DCIS, 1 case mucinous carcinoma, 2 cases of severe atypical hyperplasia; B--ultrasonic and mammography--negative 25 cases of female breast MRI examination, we found one case of DCIS and two cases of severe atypical hyperplasia. Three cases of breast cancer surgery in 2 cases Mediator, modified radical mastectomy in 1 case, 4 cases of atypical hyperplasia underwent lobe resection. The B--ultrasonic and mammography or both joint inspection negative young women implement breast MRI, can significantly improve early breast cancer detection rate, thus contributing to improve breast--conserving rate.
