老年人胰十二指肠切除术30例临床分析

摘　　要：目的:探讨胰十二指肠切除治疗老年胰头及壶腹周围肿瘤的可行性及围手术期特点。方法:对63例胰头及壶腹周围肿瘤患者行胰十二指肠切除术,其中年龄≥65岁30例(老年组),<65岁33例(对照组),对其临床资料进行回顾性分析。结果:老年组术前合并症(主要是合并心肺疾病)的发生率明显高于对照组,两组比较差异有统计学意义(P<0.05);老年组术后并发症发生率为40.0%,对照组为30.3%,两组比较无统计学差异(P>0.05)。结论:老年患者术前合并症发生率较高,但其术后并发症的发生率并无明显增加,积极的围手术期处理是减少并发症的关键因素,胰十二指肠切除治疗老年胰头及壶腹周围肿瘤是安全可行的。Objective： To discuss the feasibility of pancreaticoduodenectomy for the aged and the character of peroperative period.Methods： 63 cases performed pancreaticoduodenectomy were analyzed by retrospective research.Results： The incidence rate of the combined disease in the aged group,especially vascular and pulmonary disease was higher than that in the control group.There was significance difference between the two groups（P〈0.05）.There was no significance difference in the complications between the two groups after operation（P〉0.05）.Conclusion： The aged have the higher incidences in the combined disease,but there is no significant increase in the complications through positive preparation.Pancreaticoduodenectomy is safety and feasibility for the aged with the head of pancreas and periampullary tumor.
