DOF方案新辅助局部晚期胃癌疗效观察

摘　　要：目的评价DOF方案新辅助化疗局部晚期胃癌的临床疗效。方法回顾性分析70例进展期胃癌患者采用DOF方案新辅助化疗的临床治疗效果。结果 5例患者予DOF方案后完全缓解,43例患者部分缓解,此部分患者行根治性手术切除,其中达R0切除的30例,R0切除率为70%。19例患者予DOF后病情无明显变化,其中11例子患者行姑息性手术治疗。2名患者病情进展。结论与MAGIC试验中ECF方案相比较,DOF在疗效上不弱于ECF,而毒副作用除神经毒性发生率略高以外,其余均较ECF大大减轻。因此DOF方案可作为一种有效的局部晚期胃癌患者新辅助化疗方案。Objective To evaluate the treatment effect of DOF neoadjuvant chemotherapy for locally advanced gastric cancer. Methods A retrospective analysis was undertaken of 70 advanced gastric cancer patients who were treated with DOF neoadjuvant chemotherapy regimen. Results Five patients had complete remission, 43 patients had partial remission, 19 patients had no significant change, and two patients had progressed after DOF treatment. CR or PR patients were treated with radical surgery. Among of them, the percent of R0 resection was up to 70（30 patients）. Among 19 patients who had stable disease, 11 underwent palliative surgery. Conclusion Compared with ECF regimen in MAGIC trial, DOF is weaker in therapeutic effect than ECF for markedly reducing side effects except the neurotoxcity. On the whole, DOF can be an alternative neoadjuvant chemotherapy regimen in locally advanced gastric cancer patients.
