后程加速超分割三维适形放疗食管癌的临床研究

摘　　要：目的探讨食管癌后程加速超分割三维适形放疗的疗效及其预后相关因素。方法回顾分析后程加速超分割三维适形放疗67例食管癌患者的临床资料,分析生存率、不良反应、死亡原因及预后影响因素。结果中位随访时间16月。1、2、3、4年生存率分别为67.6%、50.9%、45.8%和35.9%,中位生存期为28月。Ⅰ～Ⅲ期食管癌1、2、3、4年生存率分别为76.9%、63.6%、57.3%,44.9%。单因素分析结果显示肿瘤部位、肿瘤长度、TNM分期对生存率有影响。Cox回归模型多因素分析结果表明肿瘤部位、肿瘤长度、TNM分期是独立预后因素,而性别、年龄与预后无关。死亡原因主要是局部未控或复发、淋巴结转移以及远处转移。结论后程加速超分割三维适形放疗食管癌患者的局部控制率和生存率较常规治疗的历史水平明显提高,肿瘤部位、肿瘤长度、TNM分期是影响预后的独立因素。Objective To investigate the curative effect and prognostic factors of late course accelerated hyperfractionated three-dimensional conformal radiotherapy（LCAH-3DCRT） in patients with esophageal carcinoma.Methods The clinical data of 67 cases of esophageal carcinoma treated by LCAH-3DCRT were analyzed retrospectively.The clinical data included survival rate,adverse reaction,causes of death and prognostic factors.Results The median time of follow-up was 16 months.The 1-,2-,3-,and 4-year survival rates were 67.6%,50.9%,45.8%,35.9%,respectively.The median survival time was 28 months.The 1-,2-,3-,and 4-year survival rates of Ⅰ～Ⅲ esophageal carcinoma patients were 76.9%,63.6%,57.3%,and 44.9%,respectively.Single factor analysis showed that surival rates were affected by tumor site,tumor length,and TNM staging.Cox regression multivariate analysis showed that tumor site,tumor length,and TNM staging were independent prognostic factors,while gender and age were not ralated to prognosis.The main causes of death were local failure or recurrence,lymph node metastasis and distant metastasis.Conclusions Local control and survival rates of esophageal carcinoma patients treated by LCAH-3DCRT are higher than those with conventional treatment.Tumor site,tumor length and TNM staging are independent prognostic factors.
