急性皮质下脑梗死后血管性认知功能障碍的影像学相关研究

摘　　要：目的探讨血管性认知功能障碍与急性皮质下脑梗死病灶部位、体积、数目的关系。方法 182例急性皮质下脑梗死患者根据VCI诊断标准分为:有VCI发生组81例,无VCI发生组101例。对182例患者进行一般状况调查、简明精神状态量表(mini mental state examination,MMSE)评分、颅脑MRI等检查。所获资料输入Excel表建立数据库,采用SPSS11.5软件对数据进行统计分析。结果在VCI组中,皮质下白质顶叶、侧脑室体旁白质前部、内囊前肢、内囊膝部、丘脑、左侧大脑半球的梗死发生率高于无VCI组;顶叶皮质下白质、侧脑室体旁白质前部、内囊前肢、内囊膝部、丘脑平均梗死灶数目多于无VCI组;梗死体积大于无VCI组,有统计学差异。MMSE评分与梗死体积呈显著负相关。结论顶叶皮质下白质、侧脑室体旁白质前部、内囊前肢及左侧半球梗死更易导致VCI的发生;内囊膝部多个梗死灶数目的累积与VCI的发生密切相关。VCI的严重程度与梗死体积呈正相关。Objective The infarction focus,size,amount of acute cerebral infarction in sub-cortex related to vascular cognitive impairment（VCI）.Methods Selecting 182 patients diagnosed suffering from acute cerebral infarction by diagnostic classification of vascular cognitive impairment（VCI）,81 patients with VCI,while another 101 cases without VCI served as control group.Using uniform questionnaires,research objects were studied by doctors with training.Questionnaires include common conditions,MMSE score result,cerebra MRI and so on.Input the data obtained into Excel to establish a database,and doing statistical analysis with SPSS11.5 software.Results In the VCI group,in parietal lobes of white matter in sub-cortex,anterior of white matter in cella lateralis paraphysis,anterior limb of internal capsule,genu of internal capsule、thalamus and left hemisphere,the incidence rate of infarction is higher than the group without VCI;In the VCI group,in parietal lobes of white matter in sub-cortex,anterior of white matter in cella lateralis paraphysis,anterior limb of internal capsule,genu of internal capsule and thalamus,amount of infarction focus is more than the other group without VCI.In the VCI group,infarction size is larger than the other group without VCI,there is statistical difference.Positive correlation is found between the severity of VCI and the infarction size.
