摘  要:目的探讨丙泊酚后处理对大鼠肝缺血再灌注损伤的影响。方法成年健康Wistar大鼠60只,随机分为5组(n=12):假手术组(S组)、缺血再灌注组(IR组)、丙泊酚1、2、4mg/kg后处理组(P1、P2、P4组)。用逆转录-聚合酶链反应(RT-PCR)法测定肝组织中TLR4 mRNA、NF-κB mRNA的表达,放射免疫法测定血清TNF-α的浓度,光镜观察肝组织形态学变化。结果与S组比较,各组TLR4 mRNA、NF-κB mRNA表达上调,TNF-α升高,差别均有统计学意义(P<0.01);与IR组比较,P1、P2、P4组TLR4 mRNA、NF-κB mRNA表达下调,TNF-α降低(P<0.01);与P4组比较,P1、P2组TLR4 mRNA、NF-κB mRNA表达下调,TNF-α降低(P<0.01);与P1组比较,P2组TLR4 mRNA、NF-κB mRNA表达下调,TNF-α降低(P<0.01)。病理检查显示P1、P2、P4组肝组织结构损伤明显小于IR组。结论丙泊酚后处理减轻大鼠肝缺血再灌注损伤的机制可能与下调TLR4、NF-κB的表达,降低肝组织炎症反应有关。Objective To investigate the effects of propofol postconditioning on the ischemia reperfusion injury in rat livers.Methods Sixty Wistar rats were randomly divided into five groups(n=12): sham operation group(S group),ischemia-reperfusion group(IR group),postconditioning group with 1,2,4mg/kg propofol(groupP1,P2,P4).Expression of Toll-like receptor 4 mRNA and NF-κB mRNA in liver tissues was assessed by Reverse transcription-Polymerase chain reaction,concentration of TNF-α in the serum was determined using radio-immunifaction method,morphologic changes of the livers were examined.Results The expression of TLR4 mRNA and NF-κB mRNA and serum TNF-α concentration were significantly higher in IR,P1,P2 and P4 group than in S group,the difference between groups has statistical significance(P0.01).The expression of TLR4 mRNA and NF-κB mRNA and TNF-α were significantly lower in P1,P2 and P4 group than in IR group(P0.01).Compared with P4 group,the expression of TLR4 mRNA and NF-κB mRNA and TNF-α in P1 and P2 group were lower(P0.01).Compared with P1 group,the expression of TLR4 mRNA and NF-κB mRNA and TNF-α in P2 group were lower(P0.01).Conclusion Propofol postconditioning ameliorate through inhibition of TLR4 and NF-κB activation and inflammatory response of liver tissue.
