摘 要:目的探讨p14ARF、MDM2和p53蛋白在子宫内膜样腺癌中的表达及其相互关系,为阐明子宫内膜样腺癌的发病机制、临床病理诊断和基因治疗提供理论依据和实验基础。方法采用免疫组化S-P法检测15例正常子宫内膜组织、18例子宫内膜增生组织及43例子宫内膜样腺癌组织标本中p14ARF、MDM2和p53蛋白的表达情况。结果 p14ARF蛋白在子宫内膜样腺癌组织中的表达显著低于子宫内膜增生和正常子宫内膜(P<0.05),MDM2蛋白在子宫内膜样腺癌中的表达有高于子宫内膜增生和正常子宫内膜(P<0.05),p53蛋白在子宫内膜样腺癌组织中的表达显著高于子宫内膜增生和正常子宫内膜(P<0.05)。在子宫内膜样腺癌中,p14ARF与MDM2蛋白的表达无相关性(P>0.05),而p14ARF与p53蛋白的表达呈负相关(r=-0.414,P=0.018),MDM2与p53蛋白的表达呈正相关(r=0.426,P=0.016)。结论 p14ARF蛋白失表达及MDM2和p53蛋白过表达在子宫内膜样腺癌的发生、发展中发挥着一定的作用。p14ARF和p53蛋白表达负相关,MDM2与p53蛋白表达正相关,提示三者可能在同一"p14ARF-MDM2-p53"通路中发挥作用,三者的联合检测可能有助于子宫内膜样腺癌的临床病理诊断,并为基因治疗提供可能的参考指标。Objective To investigate the expression of p14ARF,MDM2 and p53 proteins and their relationship in endometrioid adenocarcinoma,to elucidate the carcinogenetic mechanism of endometrioid adenocarcinoma and provide theoretic foundation for the diagnosis and genic therapy. Methods The expression of p14ARF,MDM2 and p53 proteins in 15 cases of normal endometrium and 18 cases of endometrial hyperplasia lesions and 43 cases endometrioid adenocarcinoma were detected with the S-P immunohistochemical method. Results In endometrioid adenocarcinoma tissues,the positive rates of p14ARF protein expression were significantly lower than that of endometrial hyperplasia and normal endometrium tissues(P0.05),but the positive rates of MDM2 and p53 protein expression were significantly higher than that of endometrial hyperplasia and normal endometrium tissues(P0.05).In endometrioid adenocarcinoma,the expression of p14ARF protein were not correlated with the expression of MDM2 protein(P0.05),but the expression of p14ARF protein were negatively correlated with the expression of p53 protein(r=-0.414,P=0.018),the expression of MDM2 protein were positively correlated with p53 protein(r=0.426,P=0.016). Conclusions The results suggest that absent p14ARF and MDM2,p53 protein over-expression may play an important role in the genesis and development of endometrioid adenocarcinoma.The expression of p14ARF protein was strongly associated with the expression of abnormal MDM2 and p53 protein.There may be a synergetic action among these genes in "p14ARF-MDM2-p53"pathway.Bound detections of p14ARF,MDM2 and p53 protein have positive effect on clinical pathological diagnosis,prognosis assessment of endometrioid adenocarcinoma,and provide genic treatment with indicators.
