中枢神经细胞瘤11例临床病理及免疫组化特点

摘　　要：目的提高对中枢神经细胞瘤（CNC）的临床病理及免疫组化特征的认识，减少漏诊和误诊。方法对11例CNC进行临床病理分析及免疫组化染色。结果11例CNC的组织学特点：肿瘤由密集均匀一致的少突胶质细胞瘤样小圆细胞和成片的无核纤细原纤维岛间质组成。未见异形增生及坏死。免疫组化11例Syn均阳性（l00％）；NSE3／11例阳性（27．3％）；Leu7l／11例阳性（9．1％）;GFAP和S100蛋白肿瘤内散在星形细胞阳性;NF和CgA阴性。结论CNC是好发于脑室内的分化好的神经元性肿瘤，病理形态易误为少突胶质细胞瘤、室管膜瘤及神经母细胞瘤。免疫标记Syn和GFAP在CNC的诊断与鉴别诊断中起重要作用。To increase one's knowledge of the clinicopathological and immunohistochemical characteristics of cen-tral neurocytoma (CNC) to decrease the rate of misdiagnosis. Methods The clinical features of 11 cases with CNC were ana-lyzed and their specimens were investigated under a light microscope immunhistochemically. Results Histologically, the tumorin 11 cases had the benign histological appearance, the oligodendroglioma-like pattern with honeycomb appearance and the charac-teristic cell free islands; no dysplasia, mitosis or necrosis was seen in the tumors.Immunohistochemically,all 11 cases expressedSyn (l00% positive), while 3 cases showed NSE positive and one case Leu7 positive. The proteins of GFAP and S-100 were i-dentified in scattered stellate tumor cells. Chromogranin A and neurofilament were usually absent.Conclusions Central neuro-cytoma is a kind of benign nervous system neoplasms located in the lateral ventricle. Its differential diagnosis excludes oligoden-droglioma,ependymoma and neuroblastoma.Positive Syn and GFAP staining results can be used as the specific markers for the di-agnosis of CNC.
