摘 要:目的应用表面增强激光解析离子化飞行时间质谱(SELDI—TOF—MS)技术检测食管鳞状细胞癌患者血清蛋白指纹图谱,筛选候选肿瘤标志物并建立诊断模型,初步探讨所建的诊断模型在食管鳞癌诊断中的临床意义。方法144例经手术后病理证实的食管鳞状细胞癌患者和50例健康人血清,用SELDI—TOF—MS检测各血清标本获得血清蛋白指纹图谱。用Biomarker Wizard筛选差异峰。扩大样本量,采用SPSS12.0中ROC曲线分析入选差异峰用于食管癌诊断的灵敏度和特异性。结果在分子量1500~30000范围内,共检测到93个蛋白峰,其中10个差异峰有统计学意义(P〈0.01)。ROC曲线面积超过08的有2768、2745、3403、6638、5972、6440、3321和3978m/z差异峰。当差异峰3403m/z以3.0为临界值时,其敏感性和特异性分别为87.5%和94.0%;3978m/z以2.0为临界值时,其敏感性和特异性分别为8417%和72.0%。其诊断价值均超过CA724、CEA和CA242单独或联合检测。结论3978和3403m/z两个蛋白对区分食管鳞癌患者和健康人群具有较好的诊断价值。Objective To detect the serum protein profile in patients with esophageal squamous cell carcinoma by surface-enhanced laser desorption/ionization time-of-flight mass spectrometry (SELDI-TOF-MS) technique. Methods SEL- DI-TOF-MS was used to detect the serum proteomic patterns of 144 cases of esophagus squamous cell carcinoma and 50 healthy subjects. Biomarker Wizard software was used for screening differential protein peaks between cancer patients and controls. The sensitivities and specificities of protein peaks were valuated by the areas under ROC curve (AUC) of SPSS software. Results Ninety three protein peaks were detected at the molecular range of 1 500 to 30 000, among which 10 were significantly different between esophagus carcinomas and controls. The AUCs of protein peak 2 768 m/z, 2 745m/z, 3 403m/z, 6 638m/z, 5 972m/z, 6 440m/z, 3 321m/z and 3 978m/z were more than 0.8. The sensitivity and specificity of 3403m/z were 87.5% and 94.0% with the cut-off of 3.0; the sensitivity and specificity 3978m/z were 84.7% and 72.0% with the cut-off of 2.0. The diagnostic values of these two protein peaks were better than serum CA724, CEA and CA242 alone or combination. Conclusion The protein peaks 3 978 and 3 403m/z detected in this study by SELDI-TOF-MS technique may used for distinguish serum samples of esophagus squamous cell carcinomas from normal subjects.
