内源性神经肽与肝硬化腹水的形成

摘  要:目的:探讨肝硬化患者血浆亮啡肽(LENK)、神经肽Y(NPY)的变化规律及其在腹水形成中的意义。方法:用放射免疫法检测19例有腹水、18例无腹水的肝硬化患者血浆LENK、NPY含量,并以18例慢性肝炎,14例急性肝炎和10名健康者为对照。结果:肝硬化患者血浆LENK含量显著高于正常人和急、慢性肝炎(P <0.01),有腹水者显著高于无腹水者(P<0、01),而血浆NPY水平肝硬化患者明显下降(P<0.01),有腹水者下降更明显(P<0.01)。结论:肝硬化患者血浆LENK、NPY等内源性神经肽水平明显变化,且以有腹水者为甚,提示这种神经肽可能参与了肝硬化患者的高动力状态循环异常,并与腹水形成有关。Purpose: To assess the clinical elevance of plasme leucineenkephalin (LENK) and neu-ropeptide Y(NPY) in patients with cirrhosis. Methods: Levels of plasme LENK and NPY were detected with radio-immunoassay in 19 patients with cirrhosis and ascites, 18 patients with cirrhosis without ascites. 18 patients with chronic hepatitis, 14 patients with acute hepatitis and 10 healthy controls. Result: The level of plasme LENK in patients with cirrhosis was significanty higher than other groups (P<0.01), in patient with cirrhosis and ascites was significanty higher than the patients with cirrhosis without ascites ( P <0.01). The level of plasme NPY in patients with cirrhosis was significanty lower than other groups (P <0.01), in patients with cirrhosis and ascites was significanty lower than the patients with cirrhosis without ascites (P<0.01). The concentration of plasme LENK was positively correlative with serum total bilirubin (TBiL) ( γ = 0. 46, P < 0.01), but negativey correlative with serum albumin (ALB) (γ = -0.46,P<0.01) and prothrombin time activily (PA)(γ = -0.53, P<0.01). The concentration of plasme NPY was negatively correlative with (γ = -0.33, P<0.01), but positively correlative with ALB (γ = 0.31,P<0.05) and PA (γ=0.46,P<0.01). Conclusion: The changes of endogenous neorupeptide contents in plasme of the patients with cirrohsis may attribute to hyperdynamic correlation state and ascites formation in these patients.
