免疫吸附法治疗吉兰—巴雷综合征的实验性研究

摘　　要：目的探讨亲和层析技术的免疫吸附法是否能有效清除血液循环中特异性抗体,为临床应用免疫吸附法治疗吉兰—巴雷综合征提供理论依据。方法以空肠弯曲菌(CJ)脂多糖(LPS)注射于Wistar大鼠,诱发实验性周围神经病;获取免疫血浆;以LPS作为配体与Epoxy-activated sepharose 6B亲和介质偶联制作亲和层析柱;利用ELISA法检测过柱前后免疫血浆抗体滴度,并提纯抗体;再将正常大鼠血浆(对照组)、特异性抗CJ LPS-IgG(特异性抗CJ LPS-IgG组)、特异性抗CJ LPS-IgG加正常大鼠血浆(特异性抗CJ LPS-IgG加正常大鼠血浆组)、CJ LPS免疫6周后免疫血浆(CJ LPS免疫血浆组)、免疫吸附后血浆(免疫吸附后血浆组)注射于Wistar大鼠神经外膜下,注射后7 d取其坐骨神经做病理检查。结果阳性的特异性抗CJ LPS-IgG血浆流过亲和层析柱后,所有管流出液中特异性抗CJ LPS-IgG均为阴性。特异性抗CJ LPS-IgG组原纤维病变率为24.7%,明显高于对照组(4.6%);免疫吸附后血浆组原纤维病变率为13.1%,高于对照组(4.6%),明显低于CJ LPS免疫血浆组(62.5%)(P<0.01或<0.05)。结论亲和层析技术的免疫吸附法能有效清除循环抗体,将成为治疗吉兰—巴雷综合征的重要方法。Objective To investigate whether immunoadsorption therapy of affinity chromatography can remove effectively circulating antibodies,providing theoretical basis for clinical application of affinity chromatography technology for peripheral neuropathy.Methods The immunized plasma of Wistar rats was obtained from experimental allergic neuropathy induced by CJ LPS;the affinity chromatography column was made by the ligand of LPS coupling with epoxy-activated sepharose 6B and purify antibodies.The titers of antibodies for passing column samples and the immunized plasma were detected by ELISA.Histological study for sciatic nerves was performed on the 7th day after injecting perineurally with the anti-CJ LPS-IgG,anti-CJ LPS-IgG pluse normal sera,immunized plasma and the plasma passing column.Results All of the anti-CJ LPS-IgG in effluents became negtive after positive plasma passing column.The incidence of pathologic fibers in group for anti-CJ LPS-IgG（24.7%） was much higher than that of control group（4.6%）;the incidence of pathologic fibers in group after immunoadsorption（13.1%） was higher than that of control group（4.6%）,and was lower than that of the group for immunized plasma（62.5%）（P0.01 or 0.05）.Conclusion The immunoadsorption therapy of affinity chromatography is able to remove circulating antibodies,it will become effective treatment for peripheral neuropathy.
