肢端雀斑样痣性黑素瘤15例临床病理分析(附1例超微结构观察)

摘　　要：目的　进一步认识肢端雀斑样痣性黑素瘤。方法　通过临床资料及组织病理学检查进行分析 ,1例作了透射电镜观察。结果　 1 5例中 ,肿瘤侵袭深度 0 .76～ 1 .5mm(ClarkⅢ级 ) 5例 ,1 .5 1～4 .0mm(Ⅳ级 ) 8例 ,>4 .1mm(Ⅴ级 ) 2例。超微结构显示瘤细胞呈多边形或不规则形 ,核内有多个核仁 ,胞浆内有大量黑素小体 ,呈类圆形 ,并可见多个核的瘤细胞。结论　该病预后较差 ,早期诊断及手术治疗可提高患者的存活率。Objective To recognize furthermore the acral lentiginous melanoma.Methods Analysis of clinical data,histopathological examination,and a case of translucent electron microscope observation.Result The depth of tumor invasion was as follows:0.76～1.5mm(Clark Ⅲ degree) in 5cases,1.51～4.0mm(Clark Ⅳ degree) in 8cases,>4.1mm(Clark Ⅴ degree) in 2cases.The ultrastructure showed that the tumor cells were polygonal or irregular shaped,there were several nucleoli within the nucleis,and numerous ovoid melanosomes in the cytoplasma,and multinuclear tumor cells could be seen.Conclusion The prognosis is rather poor.Early diagnosis and early surgical treatment may increase the survival rate of the patient.
