榄香烯治疗恶性胸水的临床研究

摘　　要：目的:比较榄香烯和顺铂胸腔内注入治疗恶性胸腔积液的疗效及不良反应。方法:依照入选标准,选择各种恶性肿瘤所致胸腔积液患者50例。榄香烯组:尽量放尽胸水后,胸腔内注入榄香烯,每次300-400mg,同时注入利多卡因200mg,庆大霉素8万U,每周1次,治疗4周;顺铂组:方法同上,胸腔内注射顺铂80-100mg,同时静脉水化,每周1次,治疗4周。用药后每周复查彩超、肝、肾功能、血常规一次,同时记录不良反应。结果:50例均可评价疗效。榄香烯组完全缓解10例(41.67%),部分缓解11例(45.83%),无效3例(12.5%),有效率87.5%;顺铂组完全缓解6例(23.08%),部分缓解10例(38.46%),无效10例(38.46%),有效率61.54%。不良反应两组治疗后未发现肝、肾功能、血常规有异常改变。结论:榄香烯组总有效率为87.5%,明显高于顺铂组的61.54%(P<0.05)。榄香烯对肝肾功能及造血系统无不良反应,用药后的不良反应明显小于顺铂,两组间不良反应发生率有统计学差异(P<0.05)。Objective：To evaluate the efficacy and safety of elemene for malignant chest effusion.Methods： Fifty patients with malignant chest effusion were enrolled.Elemene group： After putting all chest effusion,elemene 300 to 400mg,lidocaine hydrochloride 200mg,and gentamycin sulfate 8 million units were put into the chest once a week and for four weeks;Cisplatin group： cisplatin 80mg to 100mg were put into the chest once a week and for four weeks.Results： Curative effects of 50 patients were evaluated.Elemene group： complete remission（CR） in 10 patients（41.67%）,partial responses（PR） in 12 patients（45.83%）,and no relieve（NR） were found in 3 patients（12.5%）,the recent response rate（CR＋PR） was 87.5%.Cisplatin group： complete remission（CR） in 6 patients（23.08%）,partial responses（PR） in 10 patients（38.46%）,and no relieve（NR） were found in 10 patients（38.46%）,the recent response rate（CR＋PR） was 61.54%.The side reactions in liver,kidney and blood weren＇t found.Conclusion： The recent response rate of elemene group was higher than cisplatin group（P〈0.05）.The side reactions of elemene group was lower than cisplatin group（P〈0.05）.
