胸腹主动脉瘤术中的脊髓保护

摘　　要：目的　探讨胸、腹主动脉瘤术中脊髓缺血损伤的预防措施。方法　回顾性分析我院自1992～1998年收治的11例胸、腹主动脉瘤,应用术中SEP监测脊髓功能,采用低温麻醉并将脊髓主要供血动脉回植于人造血管上的脊髓保护效果。结果　全部病人无截瘫发生。结论　术前评估脊髓缺血的可能性,术中采用积极的脊髓保护,预防脊髓发生缺血损伤,可避免截瘫的发生。Objective To explore the preventive measures of ischemic damage of spinal cord during operation on aneurysms of thoracic and abdominal aorta.Methods To retrospectively analyse 11 cases of thoracic and abdominal aortic aneurysm patients treated in our hospital from 1992 to 1995.The spinal cord function was monitored by using SEP during operation,hypothermic anesthesia was used and the preventive measures were adopted to transplant the main blood-supplying artery of spinal cord back to the artificial blood vessels.Results No paraplegia occurred in all the patients.Conclusion The ischemic paraplegia could be avoided by estimating the possibility of ischemic damage of spinal cord before operation and by conducting active spinal cord protective procedures during operation.
