摘 要:目的比较4种不同方法充填年轻患者的单根管前磨牙后的牙胶充填面积百分数(PGFA),评价各种方法的效果。方法选取13~23岁患者的离体单根管前磨牙40颗,ProTaper预备,并随机分为4组,每组10颗,分别用冷牙胶侧方加压法、热塑牙胶注射加压法(ObturaⅢ)、连续波充填法(SystemB+ObturaⅢ)及结合冷侧压法的改良连续波充填法(modified SystemB+ObturaⅢ)进行充填,然后分别在距根尖2、5、8 mm处横断,各个横断面在手术显微镜下拍照,再用图像分析软件AutoCAD2004测量每个横断面的根管面积和牙胶充填面积,从而计算出PGFA。结果距根尖2 mm处,4种充填法的PGFA差异无统计学意义(P>0.05);距根尖5、8 mm处,ObturaⅢ组、SystemB+ObturaⅢ组及改良Sys-temB+ObturaⅢ组的PGFA差异无统计学意义(P>0.05),但都优于冷牙胶侧方加压组(P<0.05)。结论对于年轻患者的单根管前磨牙的充填,与冷牙胶侧方加压法相比,ObturaⅢ、SystemB+ObturaⅢ、改良SystemB+ObturaⅢ充填法在根管的中上部更具优势,而根尖部的效果无明显差别。Objective To comparae the percentage of gutta-percha-filled area (PGFA) achived in premolars with single root canals of young patients filled with four different techniques. Methods Four groups of extracted premolars with single root canals of 13 -23 years old (totally 40, n = 10) were instrumented with ProTaper, then the root canals were filled as follows : cold lateral condensation, injectable thermplasticized gutta-percha ( ObturaⅢ), continuous wave of condensation (SystemB + ObturaⅢ ) and modified continuous wave of condensation which was combined the cold lateral condensation(modified SystemB + Obtura Ⅲ) Cross sections of each tooth were made 2,5,8 mm from the apex. The samples were prepared for microscopic analysis. Digital image measurements of the crosssectioned area of the canal and the gutta-percha were performed with image analysis program AutoCAD2004 and the PGFAs were calculated. Results No significant differences were found between the 4 different techniques at 2 mm from the apex ( P 〉 0. 05 ), At 5,8 mm from the apex, PGFA in Obtura Ⅲ, SystemB + ObturaⅢ and modified SystemB + Obtura Ⅲ produced significantly higher than that in the cold lateral condensation (P 〈 0. 05 ), but there were no significant differences among them(P 〉 0. 05 ). Conclusion For the filling of premolars with single root canals of young patients, three thermoplasticized gutta-percha techniques are more effective than cold lateral condensation in filling upper parts, but there are no significant differences on the apical third.
