门静脉螺旋CTAP三维重建的临床研究

摘　　要：目的　评价门静脉螺旋ＣＴＡＰ（Ｓ－ ＣＴＡＰ） 三维血管成像的临床价值，并比较两种三维成像方法：表面阴影显示（ＳＳＤ） 和最大强度投影（ＭＩＰ） 对门静脉显示的差别。方法　对３６ 例肝癌患者经肠系膜上动脉（ＳＭＡ） 置管行Ｓ－ ＣＴＡＰ。结果　①门静脉Ｓ－ ＣＴＡＰ三维血管成像能清楚显示门脉４ ～５ 级小分支，并显示门脉分支受压、推移、破坏、中断等多种形态改变，优于间接门脉ＤＳＡ。②ＳＳＤ 能良好显示门脉主干及１ ～２ 级大分支的立体解剖关系，ＭＩＰ则对门脉４ ～５ 级小分支和因病变引起的形态改变显示较好。结论　①Ｓ－ ＣＴＡＰ三维成像门脉显示优于间接门脉ＤＳＡ。②ＳＳＤ 能显示门脉血管树的立体观。Objective To assess the clinical utility of portal vein three dimensional reconstruction using spiral-CT arterial portography,and to compare the diagnostic value of the two kinds of 3D technique which include shaded surface display (SSD) and maximum intensity projection (MIP).Methods Thirty six patients were examined by 3D-S-CTAP,20 patients suffered primary hepatic carcinoma and 16 patients were liver metastatic carcinoma.S-CTAP were scanned after a volume of 80ml of megluming diatrizoate was injected with a power injector into SMA in all patients.Results ①3D-S-CTAP image showed the 4-5 grades branches of portal vein clearly,and showed the pad effect,destruction and deviation of distal portal vein branches,it was superior to indirect portal vein digital subtraction angiography.② SSD was better than MIP in showing the 3D anatomic relationship of portal vein truck and 1-2 grades branches,MIP could better show smaller 4-5 grade portal vein branches and their imaging changes causes by tumors.Conclusion ①3D-S-CTAP is superior to indirect DSA in showing portal vein.② SSD could show the 3D anatomic relationship of portal vein tree,and MIP is better than SSD in showing smaller branches of portal vein.
