阿立哌唑与奥氮平治疗首发精神分裂症对照研究

摘　　要：目的 探讨阿立哌唑治疗首发精神分裂症的临床疗效与安全性.方法 将78例首发精神分裂症患者随机分为两组,阿立哌唑组38例,口服阿立哌唑治疗,奥氮平组40例,口服奥氮平治疗,观察8周.于治疗前及治疗2周、4周、8周末采用阳性与阴性症状量表评定临床疗效,副反应量表评定不良反应.结果 治疗后两组阳性与阴性症状量表总分均较治疗前有显著下降（P＜0.05）,同期两组间评分差异均无显著性（P＞0.05）;治疗8周末阿立哌唑组显效率81.6%、总有效率94.7%,奥氮平组分别为80.0%、92.5%,两组差异无显著性（P＞0.05）.两组不良反应均轻微,阿立哌唑组静坐不能、体重增加、月经改变、泌乳发生率显著低于奥氮平组（P＜0.05）.结论 阿立哌唑治疗首发精神分裂症疗效显著,且与奥氮平相当,但对内分泌系统影响较小,安全性高,依从性好.Objective To explore the clinical efficacy and safety of aripiprazole in the treatment of first-episode schizophrenia. Methods Seventy-eight patients with first-episode schizophrenia were randomly divided into aripiprazole group （n=38） taking orally aripiprazole and olanzapine group （n=40） doing olanzapine for 8 weeks. Clinical efficacies were assessed with the Positive and Negative Syndrome Scale （PANSS） and adverse reactions with the Treatment Emergent Symptom Scale （TESS） before treatment and at the end of the 2nd , 4th and 8th week. Results The PANSS total scores of both groups lowered significantly after treatment compared with pretreatment （P〈 0. 05）, group comparisons had no significant differences （P〉0.05） ; at the end of the 8th week, there were no significant differences in obvious and total effective rate between aripiprazole and the olanzapine group （81.6% vs. 80.0%, 94.7% vs. 92.5%, P〉0.05）. Adverse reactions of both groups were mild, incidences of akathisia, weight gain, menstruation change and lactation were significantly lower in aripiprazole than in oianzapine group （P〈0.05）. Conclusion Both aripiprazole and olanzapine have evident effects on first-episode schizophrenia, but the former has less influence on endocrine system, higher safety and better compliance.
