聚左旋丙交酯可吸收材料在下颌骨骨折中的临床应用

摘　　要：目的探讨聚左旋丙交酯可吸收材料在下颌骨骨折中的应用。方法选取69例下颌骨骨折患者,按照固定方法不同分为实验组和对照组,实验组采用聚左旋丙交酯可吸收材料内固定,对照组采用传统钛板内固定系统。术后评价两组患者伤口恢复情况,血常规,评价咬合紊乱恢复情况及测定咀嚼效能等,实验组加做超声检查聚左旋丙交酯吸收情况。结果术后两组患者伤口愈合良好,大部分患者牙间关系恢复良好。两组患者骨折处均有骨痂形成,复诊查血结果未见异常。术后2周、1个月咀嚼效能测定,实验组低于对照组,差异具有统计学意义(P<0.01);术后3个月后,两组患者咀嚼效能差异无统计学意义(P>0.05)。术后超声可见聚左旋丙交酯接骨板缓慢吸收,1年后基本分解吸收完毕。结论聚左旋丙交酯可吸收接骨板材料骨折断端固定效果理想,组织相容性较好,术后患者无不良反应且在体内可自我降解,可以应用于下颌骨骨折内固定,值得临床推广。【Objective】 To discuss the clinical application of degradation Poly（L/Lactide） in fracture of mandible.【Methods】 Select 69 patients with mandibular fracture,divided into experimental group（36 cases） and control group（33 cases）.Experimental group use Poly（L/Lactide） absorbable fixation materials,while the control group uses the traditional titanium plate fixation system.Patients were evaluated by routine analysis of blood,wound healing and masticatory efficiency etc.Experimental group was detected with ultra sound to evaluate Poly（L/Lactide）＇s absorption.【Results】 The two groups both had a good wound healing,fracture callus formation and normal blood test.After 2 weeks and 1 month of the operation,the experimental group＇s masticatory efficiency was far lower than the control group（P 〈0.01）;But after 3 months later,the difference between two groups＇ masticatory efficiency were not significant（P 〉0.05）.【Conclusion】 The degradation Poly（L/Lactide） shows good biocompatibility,no adverse reactions and self-degradation in body,can be selectively used in mandibular fracture fixation.
