社会支持程度对豫北农村社区食管癌患者生存质量的影响

摘　　要：目的了解豫北农村社区食管癌患者的社会支持状况和生存质量(QOL),探讨通过社区护理提高患者社会支持程度改善生存质量的措施。方法便利选取位于河南豫北农村社区803例食管癌患者进行问卷调查或个人访谈。问卷由患者一般情况调查表、欧洲癌症研究和治疗组织研制的生存质量核心问卷第三版中文版(ETROCQLQ-C30V3.0)和食管癌专项量表(QLQ-OES18)以及社会支持评定量表(SSRS)组成,社会支持总分<31为社会支持差,31～41为社会支持良好,>41分为社会支持充分。结果社区食管癌患者社会支持总分为(36.40±7.49)分,高于常模([34.56±3.73)分],具有统计学意义(P<0.05)。不同社会支持程度的三组患者的总体健康水平、食欲丧失和味觉障碍等3个维度得分差别具有统计学意义(P<0.05)。结论社区食管癌患者的总体社会支持状况较好,发病时间<2年、人均月收入≤1000元者QOL与社会支持程度的关系更为密切。Objective To understand social support status and QOL （quality of life） of patients with esophageal cancer in north rural community of Henan. Methods By convenient sampling, 803 patients with esophageal cancer in north rural community of Henan were investigated, with a questionnaire composed of demographic information, EORTC QLQ-C30 V3.0, QLQ-OES18 and SSRS. The patients were divided into three groups （worse, better and best social support） according to the total score〈31, 31-41, 〉41, respectively. Results The total score of social support in these patients was higher than that in the domestic norm, the scores of QL2, AP and OESTA were significantly different among three groups, P〈0.05. Conclusion The degree of social support and QOL about patients with esophageal cancer was positively correlated. The social support status was of vital importance to QOL of patients with family monthly income under ￥1 000 and short onset within 2 years.
