老年人胃溃疡中EGF VEGF的表达及临床意义

摘　　要：目的研究表皮生长因子(EGF)和血管内皮生长因子(VEGF)在老年人胃溃疡发病中的作用,探讨其在老年人胃溃疡的发病和治疗中的意义。方法收集经胃镜检查确诊胃溃疡患者60例为病例组,同期胃镜检查为浅表性胃炎者60例为对照组,两组按年龄平均分为老年组(≥60岁)和中青年组(<60岁),分别抽取胃液及活检溃疡周边组织为样本,通过酶联免疫吸附法检测胃液中的EGF与VEGF水平,通过免疫组织化学方法检测溃疡周边组织中的EGF与VEGF表达水平,统计分析各组数据,探索老年人胃溃疡愈合与EGF及VEGF之间的关系。结果老年人病例组胃液及组织中的EGF表达水平明显低于对照组,差异有统计学意义(P<0.05),老年人病例组组织中的VEGF阳性率明显低于对照组,差异有统计学意义(P<0.05)。老年人病例组胃液及组织中的EGF和VEGF表达水平明显低于中青年病例组,差异有统计学意义(P<0.05)。结论老年人胃溃疡患者胃液及溃疡周边组织中的EGF表达下降,且溃疡周边组织中的VEGF表达亦下降,可能是老年人胃溃疡难治愈易复发的因素之一。Objective To study the changes of EGF and VEGF expression in gastric tissue and its clinical significance in elderly patients with gastric ulcer.Methods Gastric ulcer group in 60 patients,superficial gastritis as control group in 60 cased.Two groups were sub-divided into Elderly group（≥60 years old） and young group（60 years old）.The gastric juice and tissue samples around the ulcer were collected by biopsy gastric juice EGF and VEGF levels detected by enzyme-linked immunosorbent assay,of EGF and VEGF expression levels in gastric tissue samples around the ulcer by immunohistochemistry.Results EGF expression of the gastric juice and the gastric tissue around the ulcer in elderly patient group was significantly lower than the control group,P0.001.VEGF positive rate of the gastric tissue around the ulcer in elderly patient group was significantly lower than the control group,P0.001;EGF and VEGF expression levels of gastric juice and gastric tissue in elderly patient group were significantly lower than that in the young group,P0.05.Conclusion Decreased,EGFand VEGF expression of the gastric tissue around the ulcer in Elderly patient may be one of the factors of relapsing easily and difficulty curing.
