食管癌三切口手术喉返神经损伤的原因分析

摘　　要：目的对168例食管中段癌及食管上段癌三切口手术后并发症(喉返神经损伤及无损伤组)进行分析。方法所有手术均右胸,上腹,左颈部切口,胃底及食管颈部吻合。结果所有病例中,有声音嘶哑25例,肺部并发症16例(合并声嘶6例),死亡2例。结论三切口手术是食管中段癌及食管上段癌常用术式,术中解剖不清及操作不当是造成食管癌术后喉返神经损伤主要原因,喉返神经损伤明显增加术后并发症,避免喉返神经损伤可减少患者术后并发症及病死率。Objective To analyse the causes of recurrent laryngeal nerve damage caused by three-field approach operation.Methods All patients 168 cases with upper and middle esophageal carcinoma were treated by using the methods of cervical,right thoracic and abdominal approaches（three-field approach） and anastomosis between the stomach body and esophageal neck.Results Among all cases,25 cases had voice hoarseness;16 cases had lung complications（,of them,6 cases had voice hoarseness）;2 cases were died.Conclusion Three-field approach is effective operation treatment in upper and middle esophageal carcinoma disease.Unclear dissection and wrong operation are the main reasons causing the damage of recurrent laryngeal nerve during esophageal carcinoma operation.
