摘 要:目的检测胃癌组织及正常胃黏膜组织中WIF-1基因启动子区甲基化情况,并结合临床病理资料,探讨WIF-1基因甲基化状态与胃癌发生、发展的相关性。方法应用甲基化特异性聚合酶链反应(methylation—specinc PCR,MSP)检测30例胃癌组织、30例癌旁组织及10例正常胃黏膜组织中WIF-1基因启动子区甲基化情况。结果胃癌组织、癌旁组织及正常对照组WIF-1基因启动子甲基化阳性率分别为76.6%(23/30)、13.3%(4/30)和0.0%(0/10),胃癌组织甲基化率明显高于后两组,差异有统计学意义(均P〈0.05)。胃癌组织中不同年龄、性别、分化程度的WIF-1基因甲基化阳性率差异均无统计学意义;有淋巴结转移组甲基化率(90%)高于无淋巴结转移组甲基化率(50%),差异有统计学意义(P〈0.05);而Ⅲ~Ⅳ期和Ⅰ~Ⅱ期胃癌组织中WIF-1基因甲基化阳性率分别为90.5%与44.4%,差异有统计学意义(P〈0.05)。结论WIF-1基因启动子区发生高甲基化导致其表达下调或缺失与胃癌的发生、发展以及临床病理特征有着密切的关系,通过MSP法检测胃癌组织及正常组织的甲基化率,为胃癌的早期诊断及综合治疗寻找新的靶点。Objective To detect the WIF - 1 promoter methylation in gastric cancer tissue and normal gastric mucosa, and to explore its relationship with the development and progress of gastric cancer. Methods Methylation - specific polymerase chain reaction (MSP) was used to detect WIF - 1 promoter methyiation in gastric cancer and tumor -adjacent tissues from 30 gastric cancer patients, and in 10 control specimens from superficial gastritis patients or healthy volunteers. Results In gastric cancer tissue,tumor - adjacent tissue and normal control groups,the WIF- 1 gene promoter methylation positive rate was 76.6% ( 23/30 ), 13.3 % (4/30), 0 ( 0/10 ), respectively. The rate of WIF - 1 promoter methylation was significantly higher in gastric cancer tissue group than that in other two groups ( P 〈 0.05 ). No correlation was found between WIF - 1 gene promoter methylation and age, gender, or tumor differentiation in patients with gastric cancer. And in advanced gastric cancer and early gastric cancer the WIF - 1 gene methylation positive rate was 90.5 % and 44.4%, respectively (P 〈 0. 05 ). Conclusion WIF - 1 gene promoter hyperm- ethylation leading to reduced or absent expression is closely associated with the development,progress and clinical and pathological features of gastric cancer. By using MSP test, detection of the methylation rate of cancer and normal tissues may help find new targets for early diagnosis and treatment of gastric cancer.
