蒽环类药物治疗弥漫大B细胞淋巴瘤中超声心动图检测的临床意义

摘　　要：目的分别采用以阿霉素和表阿霉素为主的联合化疗方案治疗弥漫大B细胞淋巴瘤,并用检测超声心动图的方法来观察不同蒽环类药物所致心脏的毒性并加以比较。方法采用CHOP方案治疗初治的弥漫大B细胞淋巴瘤患者120例,随机分为两组,阿霉素组和表阿霉素组,各为60例,用超声心动图检测两组在接受不同蒽环类药物化疗后2个周期6、个周期左心室射血分数的变化并加以比较。结果在化疗2个周期后,阿霉素组与表阿霉素组左心室射血分数下降超过10%的发生率分别为10%(6/60)和8.3%(5/60),两者的差异无统计学意义(P>0.05)。化疗6个周期后,阿霉素组与表阿霉素组左心室射血分数下降超过10%的发生率分别为23.3%(14/60)和10%(6/60),两者的差异有统计学意义(P<0.01)。结论超声心动图在对蒽环类药物所致心脏毒性的早期检测中有重要的意义,且表阿霉素所引起心脏毒性较阿霉素低。Objective To use adriamycin（ADM） and epirubicin（E-ADM）-based chemotherapy in the treatment of diffuse large B-cell lymphoma,we observed and compared the different anthracycline induced by cardiac toxicity with the detection of echocardiographic methods. Methods 120 patients with diffuse large B cell lymphoma who were initially treated were randomized into two groups： ADM and E-ADM.Both of them were used to detect and compare changes in left ventricular ejection fraction（LVEF） after chemotherapy 2 periods and 6 periods of the different of anthracycline drugs with echocardiographic methods. Results It is after 2 cycles of the chemotherapy,in doxorubicin group and epidoxombicin group the left ventricles shoot a blood fraction to descend more than 10% incidence rates to distinguish to 10%（6/60）and 8.3%（5/60）,in Both groups,the difference was not statistically significant（P0.05）.After 6 cycles of the chemotherapy,doxorubicin group and in the table with adriamycin group of left ventricular ejection fraction to fall by more than 10%,the incidences were 23.3%（14/60） and 10%（6/60）.The differences have a statistical significance（P0.01）. Conclusion Echocardiography induced by drugs to the heart of the toxic anthracycline-based early detection is of important significance,and table Epidoxorubicin caused by heart is doxorubicin low toxicity.
