青少年近视配戴渐进多焦点眼镜其屈光度变化相关性研究

摘　　要：目的观察渐进多焦点近视镜片减缓青少年近视发展的效果。方法 8～12岁青少年近视患者131例(262只眼),其中男性70例,女性61例,等效球镜度范围-0.50 D～-4.00 D。随机分为4组,研究组共3组分别配戴下加+2.50 D、+2.00 D和+1.50 D渐进多焦点眼镜;对照组配戴单光镜片。戴镜后每6个月复查屈光度、曲率、眼轴、眼压、远用镜MEM和下加MEM等,将18个月后复查结果进行统计学分析。结果与戴镜前结果比较,戴镜18个月后各组的等效球镜度、眼轴长和眼压均明显增长,差异有统计学意义;各组的曲率均无明显差异;下加+2.00 D和下加+2.50 D组的远用镜MEM和下加MEM均无明显差异,单光组远用镜MEM亦无明显差异;下加+1.50 D组远用镜MEM增加,差异有统计学意义,下加MEM无明显差异。戴镜18个月后,单光组眼轴长度增长小于3个研究组,差异有统计学意义;4组的等效球镜度的变化值、曲率的变化值、眼压的变化值以及远用镜MEM的变化值之间差异无统计学意义;3个研究组的下加MEM的变化值之间的差异亦无统计学意义。结论渐进多焦点镜片未能有效地控制青少年近视的进展和眼轴长度的增长。Objective To investigate the effect of progressive addition lenses on myopia in chinese children juvenile.Methods 131 cases(262 eyes) of 8~12 years-old patients with myopia ranged from-0.50 D to-4.00 D were randomly divided into four groups.The control group wore single vision glass and the 3 experiment groups wore add +1.50 D,+2.00 D and +2.50 D progressive lenses respectively.Refraction,corneal curvature,axial length,intraocular pressure,Far(monocular estimate method,MEM) and Add MEM were measured in each patient.All the subjects were followed up every 6 months for a period of 18 months.The data on 18 months were analyzed by using SPSS.Results After wearing the glasses for 18 months,the refraction,axial length and intraocular pressure of 4 groups significantly increased comparing with the baseline examination,and corneal curvature had no distinct difference.Far MEM increased in Add +1.50D group,the difference was statistic significant,while it had no distinct difference in other 3 groups.Add MEM in 3 experiment groups were similar.The growth of ocular axial length were significantly less in control group than 3 experiment groups.The other parameters(refraction,corneal curvature,intraocular pressure and Far MEM) among the 4 groups,Add MEM among 3 experiment groups had no distinct difference.Conclusion There was no evidence that myopia progression was retarded by wearing progressive addition lenses,either in terms of refractive error or axial length.
