Tourette综合征发病危险因素的病例对照研究

摘　　要：目的探讨儿童Tourette综合征(TS)的发病危险因素,为其综合干预提供理论依据。方法选取门诊就诊的207例TS患儿作为病例组;同期就诊的264例非慢性反复性疾病,非神经精神性疾病患儿为对照组。采用病例对照研究方法,探讨TS患儿的发病危险因素。结果单因素分析结果显示,抽动障碍家族史、慢性扁桃腺炎史、高热惊厥史、母亲妊娠反应程度、病理性黄疸等与TS发病有关;多因素非条件Logistic回归分析显示,TS的发病危险因素是男性、抽动障碍家族史、慢性扁桃腺炎史、高热惊厥史、母亲重度妊娠反应、早产、低出生体质量、极低频磁场暴露(距高压或电磁场<150 m)、每周看电视时间≥20 h及经常进食西式快餐与膨化食品(OR=1.592～5.924)。结论TS的发生与性别、遗传、感染、围生期因素、极低频电磁场暴露及饮食等多种因素有关,为有效防治TS发生,应综合考虑上述因素。Objective To explore the risk factors of Tourette syndrome（TS）so as to provide theoretical evidence for scientific prevention and intervention.Methods A total of 207 Tourette syndrome cases were from the outpatients of pediatric health care department,while the control group of 264 children was from the outpatients of pediatric department and pediatric health care department.The control group had no nervous or mental illness,and other chronic or repetitious disorders.Their related factors were surveyed by using case-control study.Results The single factor logistic regression analysis showed that gender,family history of TS,chronic tonsillitis,febrile convulsions,maternal pregnancy reaction and some other factors were related to the occurrence of TS.Data from multiple factor logistic regression analysis indicated that male,family history of TS,chronic tonsillitis,febrile convulsions,maternal pregnancy reaction（severe）,premature,low birth weight,exposure to very low frequent electromagnetic field（distance from high pressure or magnetism field 150 m）,the time of watching TV every week（≥ 20 h）,taking Western Snack and puffed food were the risk factors of TS（OR = 1.592-5.924,respectively）.Conclusions Gender,genetics,infection,the perinatal period factors,electromagnetic field exposure,diet factors are related to the occurrence of TS.The above factors should be taken into account for prevention and intervention of TS.
