摘　　要：报告1例成人巨细胞成纤维细胞瘤。患者男,72岁。8年前无明显诱因左下肢出现红色皮下结节,有轻度压痛,7年前经组织病理诊断为软组织来源恶性肿瘤,未予治疗。皮损缓慢增大,并形成多个外生性结节、肿瘤,皮损面积约8 cm×10 cm。再次行组织病理学检查,结果示真皮和皮下胶原间弥漫性瘤细胞增生。肿瘤细胞为梭形,部分细胞体积巨大或呈多核形态,细胞异形性明显。肿瘤局部有黏液沉积,血管周围淋巴细胞浸润。免疫组化示肿瘤细胞波形蛋白阳性,角蛋白AE1/3、S-100蛋白、结蛋白、肌动蛋白、平滑肌肌动蛋白及CD31均为阴性。CD34染色示第1次切片肿瘤细胞阴性,第2次切片浅部肿瘤细胞阳性,深部肿瘤细胞阴性。结合临床、组织病理及免疫组化特点诊断为成人巨细胞成纤维细胞瘤。A case of adult giant cell fibroblastoma is reported. A 72-year-old male patient presented with 8 year history of subcutaneous nodules with slight tenderness on his left lower leg. It was biopsied and diagnosed as malignant neoplasm of soft tissue and not treated in the past years. The lesion enlarged gradually and formed some exogeneous nodules and tumors and measured 8×10 square centimeter. Another biopsy was performed and it showed disseminated neoplasm ceils proliferation between the collagen. The neoplasm cells were of spindle appearance, some of them showed giant appearance with multiple nuclei and polymorphism, which was prominent. There was mucin deposition in local areas and perivascular lymphocytes infiltration. Immunohistochemsitry results showed the neoplasm was negative for keratin AE1/3, S100, desmin, actin, smooth muscle actin and CD31. CD34 staining of the first biopsy showed the neoplasm cells were negative, while CD34 staining of the second biopsy showed the upper part was positive while the lower part was negative. It was diagnosed as adult giant cell fibroblastoma according to the clinical, histopathological and immunohistochemical characters.
