垂直生长型安氏Ⅱ类1分类青少年与成人拔牙矫治前后侧貌美学改变的相关性分析

摘　　要：[目的]研究垂直生长型安氏Ⅱ类1分类病例侧貌美学指标在矫治前后改变的相关性分析,以期为正畸临床治疗方案的制定和治疗方法提供参考。[方法]选择38例垂直生长型安氏Ⅱ类1分类拔牙病例,在矫治前及矫治后拍摄头颅侧位片,运用5项美学指标进行评价青少年与成人矫治前后的美学改善效果以及相关性分析。[结果]青少年组在治疗后Y﹤0者占纳入样本的85℅,前后比较差异有统计学意义;成人组在正畸治疗后值Y﹤0者占其纳入样本的61.1℅(P﹤0.001);青少年的侧貌美学指标与多项软硬组织测量项目具有相关性,而成人的侧貌美学指标主要只与前牙以及上下唇测量项目具有相关性。[结论垂直生长型Ⅱ类1分类青少年与成人病例通过正畸治疗后侧貌美学评价都有一定的改善,鼻唇颏关系基本上得到了更好的协调,同时青少年与成人软组织的适应改建能力有所差异,青少年侧貌在正畸治疗后能得到更大的改善,提示垂直生长型Ⅱ类1分类拔牙病例在早期治疗更能够改善侧貌美观。[Objective]To evaluate the orthodontic treatment of patients with Class II division 1 malocculsion with vertical growth pattern;and to analyze the dependability of the improvement of profile esthetic index,to provide experimental results to help to make orthodontic treatment plan and treatment.[Methods]38 patients with Class II division 1 malocculsion with vertical growth pattern were included in our study.Their pre-and post-treatment cephalometric X-rays were measured.[Results]85% of the adolescent had Y﹤0 after the treatment;61.1% of the adult had Y﹤0 after the treatment;The profile aesthetic index and hard and soft tissue measurements of the adolescent were relevant,while the adult only had the front teeth and upper-lower lip measurements on the relevance.[Conclusion]Clinically,the orthodontic treatment results of the patients with Class II division 1 malocculsion with vertical growth pattern are treated effectively,and the aesthetic evaluation of these patients are remarkably improved.Nose lips chin relations are essentially coordinated;But adolescent facial profile during orthodontic treatment can be more improved,suggesting that early treatment can improve the facial profile more attractive in the cases with Class II division 1 malocculsion with vertical growth pattern.
