乙酰肝素酶在胆管癌组织中的表达及意义

摘　　要：目的探讨乙酰肝素酶(Hpa)在胆管癌组织中的表达及其与临床病理因素的关系。方法免疫组织化学染色检测59例胆管癌手术标本和同时随机选取其中经HE染色、病理组织检查确定为癌旁组织26例的Hpa表达情况,与临床病理因素进行相关性分析。结果 Hpa在59胆管癌组织中阳性表达48例,阳性表达率为81.3%,TNM分期Ⅰ期+Ⅱ期、Ⅲ期+Ⅳ期阳性率分别为77.8%、87.0%,但差异无统计学意义(P>0.05),癌旁组织中有3例表达弱阳性,阳性率表达率为11.5%。胆管癌组织及癌旁组织阳性表达率比较差异有统计学意义(P<0.05)。相关分析认为,Hpa表达与淋巴结转移密切相关(P=0.002)。与患者的性别、年龄、肿瘤大小和分化程度无关(P>0.05)。结论 Hpa在胆管癌组织中高表达,Hpa的表达与肿瘤的淋巴结转移有关。Objective To investigate the expression of heparanase Hpa in cholangiocarcinoma and its relationship with clinical pathological parameters.Methods The expression of heparanase Hpa was tested by immunohistochemical method in 59 cholangiocarcinoma specimens and 26 normal bile duct specimens.Results Hpa expression was positive in 81.3%（48/59）of cholangiocarcinoma specimens,and in 11.5%（3/26）of normal bile duct specimens.The difference between the two groups was statistically significant（P 0.05）.The positive percentage of Hpa expression inⅠ＋Ⅱand Ⅲ＋Ⅳ grade was 77.8% and 87.0% respectively,but there was no statistically significance between different TNM grade（P 0.05）.The expression of Hpa was relevant to lymph node metastasis（P =0.002）,but not to age,gender,tumor size or differentiation degree（P 0.05）.Conclusion Hpa expression was highly positive in cholangiocarcinoma,and expression of Hpa was relevant to lymph node metastasis.
