摘 要:目的探讨喉癌患者根治性治疗前后外周血CK19 mRNA、CK20 mRNA的表达及临床意义。方法采用巢式RT-PCR技术检测48例喉癌患者术前和术后6、12、24个月外周血中CK19 mRNA、CK20 mRNA的表达情况。结果 48例喉癌患者外周血术前CK19 mRNA、CK20 mRNA阳性率分别为37.5%(18/48)、45.8%(22/48);患者术后6、12、24个月外周血CK19 mRNA与CK20 mRNA的表达均较术前下降(P<0.05);复发组CK19 mRNA、CK20 mRNA术前表达均高于无复发组(P<0.05);4例复发患者术前与术后6、12个月外周血CK19 mRNA、CK20 mRNA的表达差异无统计学意义(P>0.05)。结论检测喉癌患者治疗前后外周血CK19 mRNA和CK20 mRNA的表达水平有助于评估喉癌微转移及临床疗效,从分子生物学方面为喉癌局部复发的预测提供帮助。Objective To study the expressions of CK19 mRNA and CK20 mRNA in peripheral blood of patients with larynge- al squamous cell carcinoma before and after radical treatment, and its clinical signifieances. Methods The nested reverse tran- scription polymerase chain reaction ( nested RT-PCR) was used to detect the expressions of CK19 mRNA and CK20 mRNA at different stages in 48 cases of laryngeal squamous cell carcinoma. Dynamic observation of the expressions of CKI9 mRNA and CK20 mRNA was applied within 24 months after operation. Results The positive expressions of CK19 mRNA and CK20 mRNA in peripheral blood at prior-treatment stage of 48 eases were 37.5% (18/48) and 45.8% (22/48) separately. The positive expressions of CK19 mRNA and CK20 mRNA were decreased statistically within 24 months after treatment ( P 〈 0. 05 ), but these phenomena did not show in the cases with local recurrence. The positive expressions of CK19 mRNA and CK20 mRNA at prior-treatment in the patient with local recurrence were statistically higher than that without local recurrence. Conclusion The dynamic detections by nested RT-PCR for CK19 mRNA and CK20 mRNA in peripheral blood of patients with laryngeal squa- mous cell carcinoma at prior-and post-treatment stages could be used to evaluate micrometastasis and clinic treatment, and to predict the risk of local recurrence in the level of molecular biology.
