摘  要:目的观察颅内血肿清除术结合醒脑静与保守治疗对高血压脑出血患者血脑屏障(blood-brain barrier,BBB)及S100蛋白和神经元特异性烯醇化酶(neuron specific enolase,NSE)的影响及临床意义。方法观察脑出血患者常规保守治疗组20例和常规治疗+颅内血肿清除术结合醒脑静组20例,对照分析其BBB指数和S100蛋白、NSE的变化。结果治疗组的BBB指数明显低于常规保守治疗组(P<0.05);颅内血肿清除术结合醒脑静组血清S100蛋白浓度、NSE明显低于对照组(P<0.01)。结论脑出血增加血脑屏障的通透性,使BBB指数和血清中S100蛋白、NSE增高,颅内血肿清除术结合醒脑静治疗可以减轻细胞毒性对血脑屏障的损伤,保护脑细胞,减轻脑水肿。Objective To investigate the effect of minimally invasive surgery and Xingnaojing injection on the blood brain barrier (BBB), S100 protein and neuron specific enolase(NSE) in patients with cerebral hemorrhage. Methods Difference of the BBB index and S100 protein concentration in blood serum was assessed between 20 cases of conservative treatment group and 20 cases of minimally invasive surgery group. Results The BBB index of minimally invasive surgery group was significantly lower than that of conservative treatment group(P〈 0.05). The S100 protein concentration and NSE in blood serum of minimally invasive surgery group was significantly lower than that of the conservative treatment group (P 〈 0.01 ). Conelusion The permeability of BBB in patients with cerebral hemorrhage are increased, the BBB index and the S100 protein concentration and NSE in blood serum in patients with cerebral hemorrhage are increased. Minimally invasive surgery can reduce the lesion of cytotoxicity to bloodbrain barrier and cerebral edema.
