摘  要:目的本研究对重症脑出血破入蛛网膜下隙行微创颅内血肿引流术的患者,采用尼莫地平注射液(尼莫同)经微创引流管灌注入血肿或脑室内的方法来防治脑血管痉挛,提高微创手术的疗效及减少并发症。方法作者对2004年5月~2011年5月间36例重症脑出血破人蛛网膜下隙行微创颅内血肿引流术的患者,随机对18例患者应用1:5尼莫地平-生理盐水液经微创引流管灌注入血肿或脑室内的方法,采用经颅彩色多普勒诊断仪(T(=D)2MHz探头探测大脑中动脉(MCA)血流速度来分析判断对脑血管痉挛的治疗效果。结果1:5尼莫地平液经微创引流管灌注入血肿或脑室内后MCA血流速度明显下降,患者未发生脑梗死并发症。结论研究证明尼莫地平液经微创引流管灌注入血肿或脑室内可明显降低脑血管痉挛的发生,该治疗方法简便、安全、可靠,为临床提供一个新的防治脑血管痉挛的途径和方法。Objective To improve efficacy of minimally invasion surgery and reduce the complications of inject nimodipine(Nimotop) through minimally invasion irrigation drainage to the hematoma or intraventricular of the patients who suffer from severe cerebral hemorrhage broken into subarachnoid space and undertake surgery of minimaUy invasion intracranial hematoma drainage, Methods Thirtysix patients who suffered from severe cerebral hemorrhage broken into subarachnoid space and undertook surgery of minimally invasion intracranial hematoma drainage were chosen from May 2004 to May 2011, and irrigation drainage to the hematoma or intraventricular was given to 18 patients. Transcranial Doppler diagnostic apparatus (TCD) 2 MHz was used to analyze blood flow velocity of cerebral vasospasm and the therapeutic effect. Results After injecting 1:5 nimodipinephysiological saline through minimally invasion irrigation drainage to the hematoma or intraventricular, the lower velocity of MCA was observed and no infarction was observed in all patients. Conclusion Infusion with Nimodipine can significantly reduce the incidence of cerebral vasospasm. The treatment is simple, safe and reliable and it provides a new method to prevent cerebral vasospasm for clinical practice.
