护理干预对晚期非小细胞肺癌放化疗患者不良反应及生活质量的影响

摘　　要：目的:探讨护理干预对晚期非小细胞肺癌放化疗患者不良反应及生活质量的影响。方法:回顾性分析2008年10月～2010年12月来我院接受治疗的78例晚期非小细胞肺癌患者临床资料,按照护理方法的不同,将其分为实验组和对照组,每组各39例,对照组采用常规护理,实验组给予整体护理干预,两周期治疗结束后对其近期疗效、不良反应、护理效果进行评价。结果:实验组和对照组患者的病情均好转,且实验组患者的好转情况较对照组明显,有效率[87.18%(34例)]较对照组[64.10%(25例)]高,血小板降低、静脉炎和肝功能损害等不良反应发生率[7.69%(3例)]较对照组[23.07%(9例)]低,差异无统计学意义(P>0.05)。实验组护理干预前后,SAS、SDS、QLQ-C30量表分值比较,差异有统计学意义(P<0.01或P<0.05)。结论:恩度联合第三代含铂化疗方案能有效治疗晚期非小细胞肺癌,较单纯的化疗方案治疗效果好,且不良反应发生率低,加强临床护理质量可以减轻患者因放化疗引起的毒性反应,提高治疗效果。Objective：To explore the influence of the nursing intervention to adverse effects and the quality of life of chemoradiotherapy patients with advanced non-small-cell lung cancer.Methods：The clinical data of 78 chemoradiotherapy patients with advanced non-small-cell lung cancer were retrospectivly analyzed which were collected from October 2008 to December 2010.They were randomly divided into two groups according to defferent nursing methods：experiment group（29 cases） and control group（29 cases）.The control group was given the routine care,while the holistic nursing intervention was perfomed in the experiment group.The short-term therapeutic effect,adverse effect and nursing effect were measured after two cycle＇s treatment.Results：The patients＇ condition had improved both in the experiment group and the control group.It had obviously improvement in the experiment group with its effective rate being 87.18%（34 cases）,whereas 64.10%（25 cases） in the control group.The incidence rates of platelets decrease,phlebitis and liver damage in the experiment group of 7.69%（3 cases） were lower than that in the control group of 23.07%（9 cases） with no statistically significant difference.The scores of SAS,SDS and QLQ-C30 scale all had statistical significance before and after nursing intervention in the experiment group（P〈0.01 or P〈0.05）.Conclusion：Endostar combined with the 3rd platiniferous chemotherapy regimen can treat advanced non-small-cell lung cancer efficiently and the treatment effect is superior to pure chemotherapy while it has lower incidence rate.It can improve the curative effect and decrease the toxic reaction side effect by strenghening quality of clinical care.
