摘 要:目的研究硝普钠对肺缺血再灌注损伤的保护作用。方法将12只成年杂交犬随机分为两组,低钾右旋糖苷(low potassium dextran,LPD)联合硝普钠(sodium nitroprusside,SNP)液组和LPD液组。在开始阻断左肺动、静脉及左主气管后,分别用LPD液或LPD联合SNP液?硝普钠,10 mg/L?灌注,灌注量50 ml/kg,测量主肺动脉压。缺血2 h循环开放前,冲洗左肺血管。循环开放后测量左、右肺动脉压。肺动脉压采用间歇测定法,每30分钟测定1次。再灌注4 h,取左肺下叶肺组织10%中性福尔马林固定后脱水、浸蜡、切片、HE染色,测定肺泡损伤指数、血管腔内径与血管外径比、左肺支气管灌洗液内中性粒细胞浓度及左肺湿干重比。结果左肺动脉血管壁水肿,明显增厚,肺动脉血管充血,有淋巴细胞贴壁现象,血管周围组织有中性粒细胞浸润,肺间质明显水肿增厚,肺泡腔内有大量的红细胞渗出,可见巨噬细胞。两组的肺泡损伤指数、血管腔内径与血管外径比、左肺支气管灌洗液内中性粒细胞数、左肺湿干重比、主肺动脉压及左右肺动脉压相比,差异均无统计学意义(P>0.05)。结论硝普钠对肺缺血再灌注损伤无明显的保护作用。Objective To study the effects of sodium nitroprusside on pulmonary ischemia reperfusion injury.Methods Twelve adult hybrid dogs were randomly divided into two groups: low potassium dextran(LPD) solution group and LPD associated with sodium nitroprusside(SNP) solution group.After blocking the left pulmonary artery,vein and main bronchus,LPD solution or LPD associated with SNP(10 mg/L) respectively infused the left pulmonary vessel with the amount 50 ml/kg,and measure the main pulmonary artery pressures.After 2 hours of the left pulmonary ischemia,the LPD solution and the LPD associated with sodium nitroprusside solution respectively washed the left pulmonary vessel.Stoped the blocking and measured the left and right pulmonary arterial pressures every 30 minutes.The pulmonary tissues were fixed with 10% formalin,after dehydrated,immersed the transparent examples in the wax,and cut into slices,baptized with HE.Determined the alveolar damage indices,the ratio of vascular inner diameter and outer diameter,the number of leukocytes in bronchus lavage solution,the ratio of the wet and dry weight.Results Hemal wall of pulmonary artery blood vessels thicken,lymphocytes congestion on the wall,neutrophils infiltrating perivascular organization,interstitial lung marked edema and thicken,the alveolar space effuse a number of red blood cells,also a small amount of the macrophages.In the two groups,the alveolar damage indices,the ratio of vascular inner diameter and outer diameter,the number of leukocytes in bronchus lavage,and the wet and dry weight ratio all had no significant difference(P〈0.05).Conclusion The sodium nitroprusside has no significantly protective effects on pulmonary ischemia reperfusion injury.
