直肠癌患者血液高凝状态的相关性研究

摘　　要：目的观察直肠癌患者与健康志愿者血小板活化状况、凝血参数、血小板相关参数,进一步明确直肠癌患者血液高凝状态.方法选择病理确诊为直肠癌的患者89例为观察组,同期来云南省肿瘤医院进行肿瘤普查的健康志愿者40名为对照组,测定外周静脉血活化血小板CD62P、CD63的表达量,以及凝血四项(TT、PT、APTT、FIB)和血小板参数(PLT、PDW、MPV、P-LCR).结果与对照组比较,观察组血小板活化标志物CD62p、CD63的表达量明显升高,FIB及血小板参数PLT、PDW、MPV、P-LCR均升高,组间比较差异有统计学意义(P<0.01);凝血参数PT、APTT、TT比较,差异无统计学意义(P>0.05).结论直肠癌患者血液的高凝状态主要是以活化血小板、血小板参数、FIB的增加为特点;活化血小板CD62P、CD63是预测直肠癌患者血液高凝状态新的敏感指标.Objective To investigate platelet mobilizing function, blood clotting function and platelet correlated parametric detection index of rectal cancer patients and healthy volunteers, to dentify correlation of rectal carcinoma and blood hypercoagulable state. Methods We selected 89 rectal cancer patients confirmed hy pathology as observation group and 40 healthy volunteers as the control. The expression of CD62P and CD63 in activated platelets, blood clotting function （TT, PT, AFIT and FIB ） and platelet correlated parameters （PLT, PDW, MPV and P-LCR） of subjects in the two groups were measured. Results Compared with health volunteers, activation markers of platelet CD62p and CD63, platelet correlated parameters PLT, PDW, MPV, P-LCR and blood clotting function correlated parameter FIB were obviously increased （p 〈 0.01 ）. The differences in TT, PT and APTT were not significant （P 〉 0.05）. Conclusions Rectal carcinoma patients usually have hypercoagulable states. The characters of hypercoagulable states are the increase of activation markers of platelet, platelet correlated parameters and blood clotting function correlated parameter FIB. Activation markers of platelet CD62p, CD63 are new senstive indexes to predict blood hypercoagulable state of rectal carcinoma patients.
