激光治疗新生血管性青光眼临床分析

摘　　要：目的　评价激光治疗新生血管性青光眼的疗效和最佳方式的选择。方法　对 3 8例新生血管性青光眼均施行巩膜Nd :YAG激光睫状体光凝术 ,其中 1 8只眼眼底清晰者配合行氩氪离子激光全视网膜光凝术 ,6只眼行氩氪激光虹膜新生血管光凝术。结果　经巩膜睫状体Nd :YAG激光光凝的患者中 1个月和 1 2个月后分别有 2 7只眼和 2 4只眼眼压等于或低于 3 87kPa。手术成功率分别为 71 %和 63 %。在 1 8只眼配合氩氪离子激光进行全视网膜光凝治疗后 1个月和大于 1 2个月的患者中 ,眼压等于或低于 3kPa者分别为 1 5只眼和 1 4只眼 ,成功率分别为 83 %和 77 7%。 6只眼虹膜新生血管氩氪离子光凝术后 1个月眼压均等于或低于 3kPa,虹膜新生血管大部分萎缩者 4只眼 ,成功率为 66 6%。患者中发生了不同程度的并发症。结论　激光光凝是治疗新生血管性青光眼的方法之一 ,但术中和术后仍有相当比例的患者会发生不同类型和不同程度的并发症。OBJECTIVE To evaluate the efficiency of laser on treatment of neovascular glaucoma.METHODS Nd:YAG laser cyclophotocoagulation through the sclera was performed in the treatment of 38 patients with neovascular glaucoma, and combined with argon krypton laser panretinal photocoagulation in the 18 eyes with a favorable visibility of fundus, and the iris neovascularization phtocoagulation in the 6 eyes. RESULTS There were the intraocular pressure ( IOP ) stabilized at 3 87 kPa or lower in the 27 eyes after treatment of Nd:YAG laser photocoagulation through the sclera at one month, and in the 24 eyes at 12 months respectively. Successful rate of operation was 71% and 63%. Fifteen eyes had the inrtaocular pressure stabilized at 3 kPa or lower at one month after treatment, and 14 eyes at 12 months or more in the 18 eye combined with argon krypton laser punretinal photocoagulation respectively. Successful rate of operation was 83% and 77 7% respectively. Iris neovascularizations were photocoagulated with argon krypton laser in the 6 eyes, and their IOP were 3 kPa or lower at one month. There was atrophy of a majority of iris neovascularization in the 4 eyes. Successful rate of the treatment was 66 6%。Some complications occurred in various degree in this patients. CONCLUSIONS Although laser photocoagulation of neovascular glaucoma may produce the complications in some patients, it is one of therapeutics.
