精索富于细胞性血管纤维瘤1例并文献复习

摘　　要：目的探讨富于细胞性血管纤维瘤的临床病理特征、治疗方式及预后。方法报道1例精索富于细胞性血管纤维瘤的临床病理特点及治疗情况,结合文献复习讨论。患者,男,49岁,因"发现无痛性阴囊肿物1月余"就诊,体检发现左侧阴囊内一实质性肿物,约4cm×2cm,表面光滑,质地韧偏硬,边界清晰,无压痛。结果行左侧阴囊内肿块切除术,镜下见肿瘤由密集的梭形细胞和丰富的血管构成,免疫组化示CD34、SMA、VIM阳性,MSA弱阳性,CK、Desmin、EMA、ER、PR均阴性。病理诊断为富于细胞性血管纤维瘤。术后随访2个月,未见肿块复发。结论富于细胞性血管纤维瘤是一种少见而独特的良性间叶性肿瘤,好发于两性生殖区,其镜下特征为密集而温和的梭形细胞及丰富的厚壁血管分布于富含纤细胶原纤维的间质中。手术切除为其最佳治疗方式,预后良好。Objective To investigate the clinicopathologic features, treatment, and prognosis of cellular angiofibroma. Methods The clinical and pathological data of one case of cellular angiofibroma of spermatic cord was reported and the relevant literatures were reviewed. A 49-year-old man presented with a painless mass in the left scrotum. Physical examination showed a well-demarcated rub- bery painless mass in the left scrotum with smooth surface,measured 4cm ×2cm. Results A complete excision of the mass was per- formed. Microscopically,the lesions were composed of spindle-shaped ceils associated with numerous small-to medium-sized blood ves- sels. The lesions stained positive CD34,SMA,VIM,weak for MSA,and negative for CK, Desmin,EMA ER and PR by immunohisto- chemistry. The pathological diagnosis was cellular angiofibroma. The patient is alive without any evidence of recurrence or metastasis after 2 months＇ follow-up. Conclusion Cellular angiofibroma represents a rare benign mesenchymal tumor, occurring mainly in the superficial soft tissue of the genital region,characterized by bland spindle-shaped cells arranged without any pattern in a stroma with wispy collagen and numerous small-to medium-sized thick-walled vessels. The best treatmeat of cellular angiofibroma is complete exci- sion,and the prognosis is well.
