伽玛刀治疗脑转移瘤的治疗策略探讨(附32例临床分析)

摘　　要：目的:探讨伽玛刀治疗脑转移瘤的临床价值和治疗策略。方法:选择脑转移瘤患者32例(72个病灶)采用伽玛刀治疗,肿瘤周边剂量12～16Gy,平均14Gy;中心最大剂量22～32Gy,平均28Gy。对32例患者伽玛刀治疗后进行临床随访,随访时间为1～20个月。结果:完全缓解5例(15.6%),部分缓解18例(56.2%),无变化7例(21.8%),进展3例(6.25%);肿瘤局部控制率为90.6%(29/32)。平均生存期11.6个月,未出现严重不良反应。结论:伽玛刀治疗脑转移瘤具有疗效好、安全的优势,能有效提高脑转移瘤患者生活质量,延长生存期。Objective： To observe the therapeutic effect of gamma knife（GK） radiosurgery on brain metastases.Method： 32 cases of brain metastases were treated with GK.The doses for the tumor margin and center were respectively 12 to 16 Gy（mean 14Gy） and 22 to 32 Gy（mean 28 Gy）.All patients were followed up after GK radiosurgery periodically.The follow up lasted for a period ranging from 1 to 20 months.Result： Of the 32 cases brain metastases who were followed up,tumor size vanished in 5 cases（15.6%）,decreased in 18 cases（56.2%）,unchanged in 7 cases（21.8%）,increased in 3 cases（6.25%）,local tumor control rate was 90.6%（29/32）.The mean survival period was 11.6 months,and most cases had no any side efects.Conclusion： GK radiosurgery is an safe and effective therapeutic method for brain metastases,and it can promote patients life quality and elongate survival span.
